Responsibilities of School. (a) School shall only assign Students to the Program who are in good standing with School, who possess a satisfactory record and who have met the minimum requirements (b) School shall require Students and Faculty Members to become acquainted with Facility’s rules, regulations, policies and procedures prior to participation in the Program or any activities at Facility and shall require Students and Faculty Members to comply with such rules, regulations, policies and procedures, including policies relating to the confidentiality of patient records, in addition to School’s own applicable policies and procedures. Facility shall assume the responsibility for informing School of any changes in its rules, regulations, policies or procedures. (c) School shall maintain overall responsibility for the curriculum, instruction, academic evaluation and related academic matters concerning Students participating in the Program. School shall provide Facility with such evaluation forms or other reports to be used by Facility, at School’s reasonable request, to evaluate the progress of each Student. School shall be responsible for assigning all final grades for Students participating in the Program. (d) School shall ensure that each Student, prior to participation in the Program, has on file with School any necessary health forms and that each Student has completed any necessary physical examinations or vaccinations that may be required by Facility or applicable law. (e) If requested by Facility, all Students and Faculty Members shall wear identification badges while at Facility denoting their status with School. (f) School shall require all Students and Faculty Members to be responsible for their own meals and other expenses while at Facility. (g) School acknowledges and agrees that Facility shall not be responsible for any salaries, taxes, insurance or other costs of Students, Faculty Members or School’s other employees or agents. School is responsible for its own debts, obligations, acts and omissions including, but not limited to, social security, health insurance, unemployment compensation, sickness and accident disability insurance, worker’s compensation and the payment of all required withholding, social security or other taxes and benefits of its Students, Faculty Members and other employees or agents, as applicable. Responsibilities of School. If it is deemed safe for the College Adviser to physically be on campus, the site will provide designated space for College Adviser Fellow(s) with access to a computer (Internet, appropriate listserv, School/district email and student information system), printer, telephone, supplies, and copiers.

School shall establish and operate a student training Program, and shall select students for the Program in accordance with all applicable state and federal law and regulations and who shall have successfully completed prerequisite courses. School shall appoint and provide the necessary faculty members who shall be both qualified and competent to provide clinical instruction. The Program shall include the academic education instruction similar to that which is provided by the School and other accredited institutions of higher learning. b. In consultation with Community, School shall be responsible for providing sufficient advance notice to Community of each student's Program schedule and any applicable academic deadlines for students. c. In consultation with Community, School shall be responsible for planning the clinical instruction component of the student training Program, as attached hereto as Exhibit A. Details of the clinical instruction plan may be modified from time to time by the Community and School to ensure that the clinical instruction plan meets the standards of the Community. d. School shall be solely responsible for instructing, guiding, and evaluating students' activities. School shall ensure that students are under the proximate supervision of a professional practitioner of School at the Community, as appropriate for the Program. School shall document progress of each student in the Program and notify the Community, upon request, of the student's progress. Students enrolled in the Program shall be subject to the authority, policies, and rules of School. e. School shall familiarize itself with the policies and facilities of the Community and shall cause its students to abide by the same. During periods of work/study assignment, students enrolled in the Program shall be subject to all rules and regulations promulgated by the Community, including but not limited to, the Sunrise Code of Conduct, health and safety rules, rules relating to patient or resident care and the privacy of patient or resident information and records, and standards of conduct and appearance. School and students shall maintain confidentiality of Community's and Sunrise's confidential business information and of patient and resident information and records, and shall cause its students and faculty to execute an agreement to maintain such confidentiality standards, in the form attached hereto as Exhibit B. Responsibilities of School. 1. School shall assume full responsibility for offering the Program. School shall have a faculty comprised of qualified instructors and administrators necessary for the provisions of the Program. In collaboration with Care Center, School will plan, conduct, and evaluate all clinical instructions and student evaluations. 2. School shall assign only those students who have satisfactorily completed the required course of study up to the current affiliation period, and who have met the standards of safety, health and academic ability as established by School and/or Care Center. 3. In addition to a current physical examination and reports on file with the School for each student, which records shall include at a minimum, evidence that each student is free from contagious disease and does not otherwise present a health hazard to Care Center residents, employees, volunteers or guests prior to his or her participation in the Program, Mantoux Test or evidence that student is free of symptoms of pulmonary disease, if the skin test is positive a chest x-ray following a positive TB test result; Hepatitis-B Vaccine or signed declination and immunization records; and negative drug screening results. The School shall ensure that all students participating pursuant to this Agreement has personal medical/accident health insurance or has signed a verification of personal responsibility for same. 4. School acknowledges and agrees that as a condition of participation by the students in the clinical experience, Care Center shall require a criminal background check of each student in accordance with any and all applicable state laws. The School shall give proof of criminal background checks conducted within thirty (30) days prior to the student’s initial assignment at the Care Center, establishing that such individual has no criminal charges currently pending and has not been found guilty of, regardless of adjudication, or entered a plea of nolo contendere or guilty to, a felony or a crime involving abuse or moral turpitude. 5. Before the start of each student’s rotation, School shall provide to Care Center information relevant to student’s clinical affiliation, covering level of training and previous clinical experience. 6. Responsibilities of School. School shall provide the most appropriate space within the school for mobile dental services rendered to ensure the highest level of infection control practices can be achieved for the health and safety of all students, staff, and visitors.

School will plan and determine the adequacy of the educational experience of the students in theoretical background, basic skill, professional ethics, attitude and behavior and shall assign to Host Agency only those students who have satisfactorily completed the prerequisite didactic portions of School’s curriculum. 2. School will retain ultimate responsibility for the education and evaluation of its students. School’s representative for this Agreement shall be a faculty member appointed and assigned by School, who will be responsible for student teaching and evaluation provided pursuant to this Agreement. 3. School will advise all students assigned to Host Agency facilities regarding the confidentiality of patient/client records and patient/client information imparted during the practice experience. School will also advise all students that the confidentiality requirements survive the termination or expiration of this Agreement. 4. School will require all participating students to maintain health insurance and provide proof of health insurance to School. Host Agency may request the student provide proof of health insurance prior to beginning of the practice experience. 5. School will require all participating students to have completed a North Carolina appropriate criminal background check, and to have documented appropriate immunizations on file with School. Specifically, Student, and any representative of School who will be on Host Agency’s campuses, must have submitted to a criminal background check and a Sex Offender Registry Check consistent with N.C. Gen. Stat. 115C-332 and -332.1. A failure to comply with these background check provisions shall be considered a default and Host Agency may immediately terminate part or all of this Agreement. If applicable, Host Agency shall notify the student of any requests for evidence of criminal background test or immunization. School will inform the student of his/her responsibility to provide evidence to Host Agency of any required criminal background checks or immunizations, when requested. Host Agency shall notify School of its requirements of an acceptable criminal background check and required immunizations. School will also inform students that they may be required to undergo a drug test or other similar screening tests pursuant to Host Agency’s policies and practices, and that the cost of any such test will be paid by the student, if not Host Agency. 6. Responsibilities of School. 1.1 School shall retain general responsibility and discretion for content and methods of instruction, supervision, control, evaluation and related matters concerning the structure of the social work field education program and student admission to, dismissal from and participation in and faculty appointments to the social work field education program. School shall be responsible for advising Facility/Agency regarding appropriate student assignments, especially as related to any known student special needs. All students shall be identified as Xxxxxxx University - Center for Social Work field education students while at Facility/Agency. The activities of the students while participating in the education program at Facility/Agency shall be the following: learning experiences deemed appropriate for the educational level of the Undergraduate and Graduate students as mutually agreed upon by representatives of the Facility/Agency and School. 1.2 School agrees to provide the services of a coordinator of social work education (Director/Assistant Director of Field Education) as liaison between School and Facility/Agency. The Director/Assistant Director of Field Education shall establish the social work education objectives for the affiliation and the methods for their implementation and for the evaluation of their effectiveness. The Director/Assistant Director of Field Education will provide Facility/Agency with necessary information, including that regarding appropriate student assignments, prior to the student's arrival at Facility/Agency and will plan appropriate visits and consultation conferences in person, by phone or via virtual technology. The Director/Assistant Director of Field Education will supply all forms necessary for Facility/Agency to evaluate students in a timely and proper fashion, if Facility/Agency is so required. Responsibilities of School. 1. SCHOOL will plan and determine the adequacy of the educational experience in theoretical background, basic skills, professional ethics, attitudes, and behaviors, and the clinical training of dental therapy students, and shall assign to TRAINING SITE only dental therapy student(s) who have satisfactorily completed the prerequisite didactic and clinical training portions of SCHOOL’s curriculum and who passed all the required course competencies. 2. SCHOOL will retain ultimate responsibility for the education and assessment of its dental therapy students. SCHOOL’s representative for this Agreement shall be a Skagit Valley College Dental Therapy Faculty member (“Faculty”), appointed, and assigned by SCHOOL, who will be responsible for dental therapy student teaching and assessment in course DT 295 (“Program Director”). 3. If requested by TRAINING SITE, SCHOOL will provide instruction to TRAINING SITE’s staff with respect to SCHOOL’s expectations regarding assessment of SCHOOL’s dental therapy students at TRAINING SITE. 4. Program Director or designated SCHOOL faculty shall meet with TRAINING SITE Clinical Education Supervisor (a.k.a. clinic Dental Director) or designated TRAINING SITE staff on a mutually agreed upon schedule or at minimum every other week. Meeting discussions include but not limited to: education program updates, education program training/informational session(s), student progress, education program employee complaint, TRAINING SITE inquiries/concerns, TRAINING SITE patient complaints, etc. 5. SCHOOL shall provide TRAINING SITE feedback based on applicable student experience assessment(s) findings and student exit interview responses.
Responsibilities of School. A. School will schedule students for the Clinical Program in collaboration with the High School and consistent with accepted nondiscriminatory practices. B. School will assign to the Clinical Program only those students who satisfactorily completed the prerequisite portions of the curriculum. C. School will provide students with information regarding expectations for the Clinical Program and with general instruction needed for students to perform specific assigned tasks. D. School agrees that each student and faculty member assigned to work with the High School is in good standing with the School and, if applicable, is licensed through the appropriate licensure board. E. School shall provide copies of records related to student and faculty members good standing with the School to the High School as requested, but, with regard to student records, only with the consent of the student. F. School agrees to provide instruction to students pertaining to blood borne pathogens and universal precautions in compliance with Occupational Safety and Health Administration Regulations. G. School shall insure itself and its employees through a fiscally sound program of self-insurance or commercial insurance of a combination thereof, for professional and general liability.
Responsibilities of School. School shall fulfill the following terms, obligations and covenants: 1. School has full responsibility for the academic content of the practicum and the credit granted for its satisfactory completion; 2. Assign a faculty member as a liaison with College to provide support of the program for students; 3. Accept the overall responsibility for the education, evaluation, qualifications, and competency development of each Student; 4. Maintain responsibility for the salary and expenses of regular School faculty providing oversight under this Agreement; 6. Inform Students of their obligation to adhere strictly to the policies, procedures, rules, standards, schedules and practices of College and its affiliated facility partners; 7. Inform Students and faculty members who are supervising Students regarding their obligation to maintain confidentiality of all College and its affiliated facility partner matters and information 9. Ensure that Students assigned to College and its affiliated facility partners under this Agreement have professional liability insurance in the minimum amount of $1,000,000 per claim and $3,000,000 in the aggregate prior to participating in clinical education experiences. This coverage may be under a blanket policy; 10. Notify students about their obligations to comply with College and its affiliated facility partners policies and procedures, state law, and OSHA bloodborne and tuberculosis pathogen regulations in the training, vaccination, testing, prevention and post-exposure treatment of Students, where applicable in the performance of duties required by the Programs. When required by College or its affiliated facility partners, notify Students of the necessity to remain current for annual tuberculosis testing; 11. Provide Students who may be involved in patient care with comprehensive infection control training, including blood borne pathogens, prior to rotation at College and its affiliated partner facilities; 12. If applicable for the Program, students will complete CPR training prior to any clinical assignments at College and its affiliated facility partners; 13. Inform students that College and its affiliated facility partners may require criminal background checks for all Students, which may include a report stating the Student has not been convicted of a felony, Medicaid fraud/abuse, or excluded/suspended from participation in the Medicare program, and which may be at the Student’s expense; 14.
