Sessions. I normally conduct an evaluation that will last from 2 to 4 sessions. During that time, we can both decide if I am the best person to provide the services you need in order to meet your treatment goals. If psychotherapy is begun, I will usually schedule one 45-minute session (one appointment hour of 45 minutes duration) per week at a time we agree on, although some sessions may be longer or more frequent. PROFESSIONAL FEES The following information pertains to my financial policy. I hope this will answer any questions that you may have, but if you do have any questions or special concerns please do not hesitate to discuss them with me at the first session. Please acknowledge your understanding of this policy by signing at the end of this form. If you would like a copy of this form for your records I will be happy to provide one for you. My fee is $175.00 for individual sessions and $185.00 for couples or family therapy sessions, paid at the end of each session. The usual therapy hour consists of 45 minutes. The fee for the initial diagnostic session is $195.00. Charges for services outside the usual therapy hour that you may request of me will be determined on an individual basis. These services might include report writing, telephone conversations (lasting longer than 110 minutes), consulting with other professionals with your permission, and the preparation of records or treatment summaries. If you become involved in legal proceedings that require my participation, you will be expected to pay for all of my professional time, including preparation, transportation, and waiting costs, even if I am called to testify by another party. Because of the difficulty of legal involvement, I charge $200.00 per hour for preparation and $350.00 per hour for attendance at any legal proceeding. BILLING AND PAYMENTS Payment isexpected at the end of each session. Please discuss any exceptional circumstances with me at the first session. Visa and MasterCard are accepted for your convenience. An insurance receipt is available should you wish to submit your insurance claims personally. If you are a member of a managed care company in which I participate, I am required to file insurance for you. Sessions. Negotiations will be conducted at times and places mutually agreeable to the negotiators named by each party. At the first meeting the negotiations teams will agree on a process for the sessions. Items to agree on may include frequency of meetings, minutes, process, agendas, tentative completion date, and other items. Negotiating sessions shall be open and publicized in order to meet the Idaho Open Meeting Law. Both parties will maintain the right to caucus in private when deemed appropriate. Minutes shall be recorded by the Board. Minutes of the previous session will be read, amended as necessary, and approved at the beginning of each session. Approval will be indicated by the signatures of the spokesperson for the Association and School Board. Copies will be provided to the Association.

Sessions. Your first session with your clinician will involve an evaluation of what problems appear most important to work on and what treatment needs you have. By the end of this first evaluation time, your clinician will give your their thoughts about your work together and what treatment they feel will be most helpful. This first session will evaluate your needs and determine if your clinician and the BHCTC is right for your treatment. This initial session will also help you to determine if you feel comfortable receiving treatment at the BHCTC. If you and your clinician choose to begin treatment together, you will be scheduled for treatment sessions at a length and frequency determined by you and you clinician. The goal of treatment is to get you feeling and functioning better as efficiently as possible and we therefore limit the total number of sessions to approximately 12 or three-months of weekly therapy sessions. You will receive the best benefit from treatment if you come to your appointments regularly. If you must cancel an appointment, this Agreement represents your promise that you will do so at least 24 hours in advance of your scheduled time. If you do not cancel 24 hours in advance, the BHCTC will need to charge you for a missed session because your clinician will have reserved that treatment time for you and will be unable to use that time to see any other patient. A missed appointment cannot be billed to your insurance carrier, which means that you will be responsible for paying the BHCTC in full a $100 cancellation fee for the missed appointment. The BHCTC recognizes that emergencies or other unplanned events do arise that cannot be helped (flat tires, sick children, family emergencies), so this Agreement allows you to cancel one (1) appointment less than 24 hours in advance within any 6 month period without any charge to you. If you cancel more than that, you understand and agree that BHCTC can bill you $100 for each missed appointment. Sessions. Meet and Confer sessions will be open to the public, unless otherwise mutually agreed upon in advance. The only people empowered to speak are the authorized Meet and Confer representatives. The public may speak, if desired, after adjournment of the regular meeting.

Our office understands that occasionally circumstances beyond your control arise. We will work with you if that is the reason for cancellation. However, only 2 less than 24-hour cancellations will be excused in any 6-month period. It is important to note that insurance companies do not provide reimbursement for cancelled sessions. PROFESSIONAL FEES. Fees vary depending on the services provided and the credentials of the counselor. A fee schedule is provided on the Professional Disclosure Statement. In addition to weekly appointments, this office charges for other professional services you may need. Other services may include report writing, telephone conversations lasting longer than 5 minutes, consulting with other professionals, preparation of records or treatment summaries, and the time spent performing any other service you may request. If you become involved in legal proceedings that require this office’s participation, you will be expected to pay for all professional time, including, but not limited to communication with any involved attorney; court testimony; and preparation; as well as any expenses or costs, such as transportation or parking. Due to difficulties associated with legal matters, this office charges a higher hourly rate for work related to legal matters. Please refer to the current fee schedule. Your counselor is often not immediately available by telephone. This may be because the counselor is seeing another patient or due to other work commitments. While your counselor will usually be in the office between 8:30 a.m. and 5:30 p.m., your counselor will not answer the phone when with other patients. Sessions. The basic unit of performing work will be a ‘session’. The length of each session will be calculated in 15 minute units. The maximum scheduled length of a session will usually be 3 hours, however dress or technical rehearsals for opera/ballet/music theatre or performances of single long works (i.e. St Xxxxxxx Passion, the Messiah) may be of 3½ hours duration. The minimum scheduled length of a session will be 1 hour. There may be more than one session scheduled in a day provided that the total performing time does not exceed 6 hours. A session may include concerts, rehearsals, media work or any combination of these. The length of rehearsals and media work will be predetermined. The duration of a concert will be estimated on the work schedule. The finishing time of a concert is defined as the time at which the leader (or equivalent) finally leaves the concert platform after the final item.
Sessions. After the mediator has been selected, the parties shall promptly agree upon a date and time for the initial conference with the mediator, but no later than thirty (30) days after the date the mediator was selected. The parties understand and agree that, besides counsel, a representative from each side with full settlement authority (subject to ratification by the governing authority of the City) will be present at all mediation conferences unless excused by the mediator. In addition, each party may bring additional persons as needed to respond to questions, contribute information and participate in the negotiation.
