Upon receipt of each of Interconnection Customer’s payments of such bills, Transmission Provider shall reimburse the applicable Interconnected Transmission Owner. Billing and Payments. I accept credit cards, debit cards, HSA cards, and FSA cards. No checks or cash are accepted. Before scheduling your first session, information for an active credit, debit, HSA, or FSA card is required and will be truncated and securely stored by a third-party bankcard processor, Pineapple Payments/CardConnect, to meet Payment Card Industry (PCI) requirements. Please be advised that your card for each session will be charged after the session has been held unless we agree otherwise. Payment schedules for other professional services will be agreed to when such services are requested. Any charges will show up on your financial statement (e.g., credit card statement) as “DC Services”. If payment after a session is not received for any reason, I will notify you of this and further sessions will not be scheduled until the balance is paid in full. If your credit, debit, HSA, or FSA card on file has expired, I require you to replace it with an active card before your next session is scheduled. If your account has not been paid for more than 60 days and arrangements for payment have not been agreed upon, I have the option of using legal means to secure the payment. This may involve hiring a collection agency or going through small claims court. If this should occur, its costs, including court costs, reasonable attorney fees, and prejudgment interest on unpaid fees, will be included in the claim. In most collection situations, the only information I will release regarding a patient’s treatment is his/her name, the dates, times, and nature of services provided, and the amount due. APPOINTMENTS Your scheduled time is reserved for you. If you arrive late, your session will likely be shortened by that amount of time and you are still responsible for paying the full session fee. Please try to arrive punctually to get the full benefit of your session. T elephone Sessions Depending on the circumstance, I might offer telephone sessions. This would be charged at the regular rate. Please be aware that the procedure code for this service is different than for an in- office appointment and insurance coverage may not be available depending on your particular plan. Billing and Payments. You shall pay all fees or charges to your account in accordance with the fees, charges, and billing terms in effect at the time a fee or charge is due and payable. Where Services are offered on a free trial basis, payment may be required after the free trial period ends, and not when you enter your billing details (which may be required prior to the commencement of the free trial period). If auto-renewal is enabled for the Services you have subscribed for, you will be charged automatically in accordance with the term you selected. Billing and Payments. The Accounts have various billing and payment structures, including varying periodic finance charges and fees. The following is information on the current billing and payment characteristics of the Accounts. Monthly billing statements are sent by the Seller to cardholders at the end of the billing period. Each month, the cardholder must make a minimum payment equal to the greater of (a) $10 plus the outstanding periodic finance charges and (b) the amount by which the then current balance exceeds the authorized credit limit. A balance of $10 or less must be paid in full. The full amount of any unpaid minimum payment required in respect of preceding monthly billing statements, will also be added to the minimum payment. A periodic finance charge is assessed on Accounts at the annual interest rates specified at the time each Account is opened and from time to time thereafter in the monthly billing statements relating to such Account. Periodic finance charges and fees are charged on balances for purchases, cash advances and fees starting from the transaction date until the date the payment is received. No interest is assessed on purchases and fees as long as the total new balance which is owed for the month is paid in full by the payment due date, and for small business credit cards only, the total new balance for the previous month was also paid in full by that month’s statement’s payment due date. The periodic finance charges for any month are calculated on a daily basis by multiplying each daily periodic finance charge bearing closing balance by the equivalent daily rate for the applicable annual rate. The average periodic finance charge on the Accounts relating to cash advances and purchases of goods and services was 18% per annum for the month of June 2019 (excluding introductory interest rates). The Seller may change the periodic finance charge on the various types of Accounts at any time by not less than 30 days’ written notice to the cardholders. The Seller may assess an annual or membership fee that varies depending on the features of the Account. Payments by cardholders to the Servicer on the Accounts are processed and applied first to the minimum payment, then to any remaining balance. Billing and Payments. Pole Owner shall send invoices to Licensee via regular U.S. Mail at the address specified below, or at such other address as Licensee may designate from time to time. Invoices for rental charges will be sent annually or semi-annually. Invoices for all Non-recurring Charges, Unauthorized Attachment Charges, and other obligations amounts due under this Agreement other than rental charges will be sent at Pole Owner’s discretion within a reasonable time. Invoices for Non-recurring Charges will provide specific identifying information pertaining to each charge. Invoices for rental charges will provide summary information only. Invoices will conform generally to the invoice template attached hereto as Exhibit , subject to change by Pole Owner, in Pole Owner’s reasonable discretion and Commission approval. Licensee may obtain additional information pertaining to charges upon written request to Pole Owner.
Billing and Payments. Unless Subscriber’s Internet Service is subject to a separate billing agreement, including but not limited to a bulk billing service agreement, Subscriber agrees to pay all charges associated with the Internet Service. If the Subscriber does not enroll in the auto-pay program, AireBeam reserves the right to charge a late fee for any payments made beyond the invoice due date. AireBeam also charges a $2.50 manual payment fee for any payments mailed in or requiring an AireBeam Representative to process the payment on the customer’s behalf. AireBeam will provide Subscriber with advance notice of any price change or fees or any new prices or fees. However, if the fee change was outside AireBeam’s control (e.g., increased governmental imposed fees or taxes), AireBeam may elect not to provide advance notice unless required by applicable law. Subscribers are billed in advance for recurring fees and charges unless otherwise agreed to, and AireBeam’s services are provided on a month-to-month basis. With limited exceptions (for example, where there is no outstanding balance on a cardholder’s credit card account), monthly billing statements are sent by the Seller to cardholders at the end of the billing period. Each month, the cardholder is required to make at least the minimum payment indicated on the monthly billing statement. The minimum payment amount is typically calculated by one of two methods: (1) where the cardholder (or if held jointly, all cardholders) on the credit card account reside(s) outside the province of Quebec, by adding the amount of any outstanding periodic finance charges plus other fees outstanding plus $10; or (2) for accounts held individually or jointly by any person resident in the province of Quebec, based on a percentage of the account’s total outstanding balance (which percentage will be gradually increasing from 2.5% to 5% between August 1, 2019 and August 1, 2024).
