Authorization Form. A properly executed copy of such “Authorization for Check-off of Dues” form for each employee for whom Union membership dues are to be deducted hereunder shall be delivered to the Company before any payroll deductions are made. Deductions shall be made thereafter only under “Authorization for Check- off of Dues” forms which have been properly executed and are in effect. Any “Authorization for Check-off of Dues” which is incomplete or in error will be returned to the Union by the Company. Thereafter the Union shall deliver to the Company any executed “Authorization for Check-off of Dues” forms under which Union membership dues are to be deducted beginning with the following month. In the event that membership dues other than those for the calendar month in which the deduction is made and initiation fees have become due and owing by an employee subsequent to the form, but prior to the first deduction by the Company there under, such membership dues and initiation fees will be deducted by the Company at the time it makes the first deduction for membership dues. The Union will notify the Company in writing, when it makes delivery of “Authorization for Check-off of Dues” forms of the amount owing by employees who executed these forms. In the case of employees rehired, or returning to work after layoff or leave of absence, who have previously properly executed “Authorization for Check-off of Dues” forms, deduction will be made for membership dues as provided herein. In cases where a deduction is made which duplicated a payment already made to the Union by an employee, or where a deduction is not in conformity with the provisions of the Union Constitution and Bylaws, refunds to the employee will be made by the Union. Dues deduction shall be remitted to the designated financial officer of the Local Union once each month within one (1) calendar week after such deductions are made. Any deductions made from subsequent payrolls in that month shall be included with the remittance for the following month. The Company shall furnish the designated financial officer of the Union, monthly, with a list of those for whom deductions have been made and amounts of such deductions. Authorization Form. ‌ Each employee, within thirty (30) days of employment, shall be provided an individual dues authorization form indicating whether the employee elects to join the Association and pay dues. Such form shall be provided by the ASSOCIATION with the original being sent to COUNTY Central Payroll, who will forward it to the ASSOCIATION along with the monthly itemized statement. An electronic copy will be retained by Human Resources. The Association shall be allowed to meet on duty with each newly hired bargaining unit members for thirty minutes within the employee’s first thirty (30) days to discuss representation at a time mutually agreed upon by the ASSOCIATION and the COUNTY.
