Extended Disability Benefits. An Employee who receives twenty-six (26) weeks of sickness and accident benefits may be eligible to receive extended disability benefits on a monthly basis. Payment shall be an amount equal to 66 2/3% of your monthly earnings as of the commencement of total disability up to a maximum of $10,000.00 per month. The amount of the monthly benefit payable may be reduced so that your income from all sources will not exceed 85% of your gross monthly earnings. The cost of the coverage of the Extended Disability Benefit shall be paid one hundred (100%) per cent by the Company as mutually agreed between the Company and the Union. Benefits are payable up to your 65th birthday or, if earlier, to the date on which you elect to receive early retirement benefits under any Employee benefit plan. The Union reserves the right to file a grievance in accordance with the grievance and arbitration procedure for decisions made under this benefit. Extended Disability Benefits. In the event of a total disability, which exceeds your maximum benefit period under the weekly indemnity benefit, you will receive a monthly income based on the Schedule of Benefits in Article III, Section 1. This extended disability benefit will be reduced by any other benefits you may be eligible to receive, such as Social Security, Workers’ Compensation or Company-sponsored pension benefits. This extended disability benefit begins the first day following the final weekly indemnity benefit and will continue until: you are no longer disabled, reach your Social Security Normal Retirement Date, die, or reach your maximum benefit period. Your maximum benefit period is based on the number of months you have been employed at the time of your LTD disability award date, reduced by the period you received weekly indemnity benefits. You are responsible to provide satisfactory proof of disability to the LTD insurance carrier, including obtaining necessary medical information from your provider/providers to support your disability initially and for any necessary re-certification as required by the carrier. Extended Disability (LTD) payments will be suspended until satisfactory proof is supplied. This long-term disability benefit will be reduced by any other benefits; you may be eligible to receive such as Social Security, Workers’ Compensation or Company-sponsored pension benefits. When Benefits Begin: LTD benefits begin after you have been continuously disabled for six months as a result of injury, illness, or pregnancy. This sixth-month period is called your waiting period. You must be disabled from the same cause throughout your waiting period. If your disability temporarily ends and you return to work for up to 30 days during your waiting period, your waiting period will be extended by the number of days you were able to work.

This provision provides an incentive for a graduated return to work to reintegrate into the workplace after a total disability by permitting you to earn income for hours worked, in addition to your disability benefits which are adjusted according to the return to work schedule. Here is how top up is calculated: An employee works 40 hours/week and receives a monthly benefit for total disability in the amount of $1800. If he returns to work gradually on the basis of two eight hour days per week over the course of a month, his benefits will be adjusted as follows: $1800 divided by 160 hours full time for a month = $11.25 per hour 2 days of 8 hours worked, over four weeks = 64 hours worked 64 hours worked at $11.25 per hour = $720.00 $1800 - $720.00 for hours worked as planned = $1080.00 paid in benefits for that month. To receive Extended Disability Benefits, you must be totally disabled so as to be unable to engage in any gainful occupation at the Company for which you are, or may reasonably become, qualified by education, training or experience. Your monthly benefit is shown in the summary of benefits. Effective December 31, 2006, employee’s age 65 or older will not be eligible for coverage under the Extended Disability Plan. If, because of differences in the respective entitlement criteria, you are eligible to receive Extended Disability Benefits hereunder and you qualify for disability benefits under C.P.P./Q.P.P., your Extended Disability Benefit will be reduced by an amount equivalent to such C.P.P./Q.P.P. benefit. Such C.P.P./Q.P.P. offset will be frozen at an amount of $414.13 per month for the life of the Collective Agreement.
