It is a self-funded plan administered by Cigna Health Care and consists of the current benefit: Cigna Pharmacy Retail Drug Plan; $15 per 30 day supply for generic drugs $40 per 30 day supply for preferred brand name drugs $60 per 30 day supply for non-preferred brand name drugs 50% of drug cost per 30 day supply for self-administered Injectables (e.g. injectables drugs used to treat rheumatoid arthritis, hepatitis C, multiple sclerosis, asthma). Cigna Tel Drug Mail Order Drug Program: $0 (no charge) per 90 day supply for generic drugs $80 per 90 day supply for preferred brand name drugs $120 per 90 day supply for non-preferred brand name drugs 50% of drug cost per 90 day supply for self-administered Injectables (e.g. injectables drugs used to treat rheumatoid arthritis, hepatitis C, multiple sclerosis, asthma). Since prescription drug costs are a major component of the health plan and are subject to significant annual cost increases, the City and the UNION agree to evaluate and measure pharmacy benefit total costs and evaluate best practice strategies to manage the pharmacy benefit. Any changes in the pharmacy benefit shall be mutually agreed to by the City and the UNION. Dental: Dental premium rates may be adjusted annually upon the City receiving notice from the dental providers. Employees will be notified of the adjustments in the dental rates during open enrollment. In accordance with current practice, when employees choose to be covered under the City’s dental plan, the employee will continue to pay the dental premium. Employee contributions: In accordance with the City’s Cafeteria Plan group health premiums will be paid by the bargaining unit employee with pre-tax dollars.
Prescription Drug Coverage. The Kawartha Pine Ridge District School Board Health Care Plan for District 14 OSSTF will contain all drugs and supplies listed in the KPR Benefits Carrier Formulary, or its equivalent, covered at 100% reimbursement through a pay- direct drug card. The KPR Benefits Carrier Formulary is reviewed and updated four times each year using an independent panel of medical experts’ recommendations on new drugs and new information on existing drugs. Note: Drugs which are not covered under the KPR Formulary Prescription Drug Plan are listed as per the master agreement between the Board and its carrier provider. A reference to drugs when administered through a hospital (whether in-patient or out-patient) will be added to the list. In the event that the Kawartha Pine Ridge District School Board Health Care Formulary for District 14 OSSTF does not contain an acceptable substitute with equivalent therapeutic value, or should the insured individual insist that a prescription be filled when a drug is not a covered expense under the plan, the plan shall provide 70% reimbursement of said prescription drug at point-of- sale, providing said prescription drug was an eligible expense under the KPR Benefits Carrier or its equivalent and providing that at least one of the following conditions is met: • There is no reasonable substitute for the drug prescribed. • There is a life threatening or debilitating disease. • There is a hazard to the person’s health. The employee will submit this information on the Drug Exception Request Application to the pharmacist at the KPR Benefits Carrier for review and the matter shall be resolved within twenty (20) instructional days of the Teacher’s initial written request. If the time line is not met, the Teacher shall be entitled to 100% reimbursement.
Prescription Drug Coverage. Prescription drug coverage will be provided to eligible employees. The prescription drug co- payment for all three (3) years of the contract will be: Mail Order - $5.00; Generic - $7.00; Name Brand - $15.00; and shall not count toward the annual deductible or be eligible for coinsurance.
Prescription Drug Coverage. The health plans described above will include prescription drug coverage with a $15 generic/$30 brand drug co-pay; and a 90 day supply co-pay of $30 generic/$60 brand name.
Prescription Drug Coverage. 199 The following schedule of prescription drug copays became effective on January 1, 2014: Effective Date Generic Preferred Brand Non-Preferred Brands 01/01/14 $10.00 $20.00 $45.00 If the University changes its prescription copays for employees not represented by a union, the University will change prescription copays for employees in the bargaining unit in the same manner and to the same extent. 200 In addition, employees will participate in the generic drug incentive program which includes use of generic medication when available and legally permitted, and where the employee or the employee’s physician requests a brand- name product when an FDA-approved generic is available, the employee will be required to pay the cost difference between the brand-name and generic medication plus the applicable brand-name copay.
