The 'Nursing Homes' clause defines the terms and conditions under which services or responsibilities related to nursing homes are addressed in the agreement. Nursing Homes a.) As soon as a supervised nursing home setting is being considered for an individual in a state hospital, the CSB shall obtain releases from the individual or his or her substitute decision maker in order to contact potential nursing homes and begin initial contacts regarding bed availability and willingness to consider the individual for placement.
b.) The state hospital will complete the UAI within five business days of the individual being found to be at Discharge Ready Level 2.
c.) Within two business days of being found to be at Discharge Ready Level 1, the state hospital will send the packet to Ascend for Level 2 nursing home screening.
d.) The CSB shall send applications to potential nursing homes identified above within two business days of the Level 2 response from Ascend.
Nursing Homes. The Facilities shall be operating only as nursing homes.
Nursing Homes. Contractor shall negotiate the reimbursement rates with nursing home providers. Contractor shall consider establish rates that consider the acuity of care provided to members as well as contain quality indicators. As a condition for payment the contractor must ensure that the Nursing Home has met all federal and state OBRA/PASRR requirements for all individuals seeking admission or readmission to a nursing home, subsequent to the provisions in 42 CFR 483.100-138 and Rhode Island Rules and Regulations for Medical Assistance Section 0378.05. The State shall approve the reimbursement method used to reimburse nursing facilities.
