Annual Performance Reports. The Organizer shall publish an annual performance report that provides the information required under IC 20-20-8 in the same manner that a school corporation publishes an annual report under IC 20-20-8 and, if the Organizer or the Charter School maintain a web site for the Charter School, both that report and the section of the Accountability Report prepared by OCS and relating to the Charter School shall be made available in a reasonably conspicuous manner on the Charter’s School’s web site. The Organizer shall notify the parents of students of the Charter School of the availability of the reports using the same method of communication used by the Charter School for other regular communications with the parents.‌

Annual Performance Reports. The Subrecipient shall be required to submit an annual “Performance Report” to the Director of Planning and Community Development, via email, at XXXX0000@xxxxxxx.xxx, which includes key performance indicators for each project, service, or deliverable, as well as both output and outcome measures. The key performance indicators and outcome measures will be mutually agreed upon by the parties. Output measures, (for example the number of students enrolled in an early learning program), provide valuable information about the early implementation stages of the project. Outcome measures, (for example the percent of students reading on grade level), provide information about whether a project, service or deliverable is achieving its overall goals. The Subrecipient must include the related outcome goal for its project, service or deliverable and provide updated information on achieving these outcome goals in its annual Performance Report during the month of August each year.
Annual Performance Reports. Each site within a portfolio of projects will have a three (3) year Performance Period during which the annual Performance payments will be calculated in the Annual Performance Report based on actual power production during the period. The required annual kWh production and corresponding annual maximum payment for each site are shown in the Payment Distribution Table at the end of the Exhibit A Statement of Work. The total production performance period of the Standard Performance Contract Agreement shall be no more than (6) years. The first year’s (consecutive 12-month) Performance Period will begin upon NYSERDA’s approval of the Project Commissioning Report. If NYSERDA determines that data collected prior to approval of the Project Commissioning Report is satisfactory, the first year’s performance period may begin at an earlier date approved by NYSERDA. Within 60 days from the end of the first year’s performance period, the Contractor must submit an Annual Performance Report to NYSERDA, which will become the basis for the first Performance payment. Annual performance reporting shall be in accordance with the approved QA/QC. Annual Performance Reports must also be submitted to NYSERDA within 60 days from the end of the second and third performance periods. The Contractor shall be responsible for ensuring that data provided in the Annual Performance Reports accurately represents the operation of the ADG-to-Electricity and ADG Production Site systems. Project Commissioning Report A Project Commissioning Report must be completed and submitted to NYSERDA within 14 months of the Effective Date; such Report must be prepared in accordance with the requirements of the approved QA/QC, and must sufficiently document that installation and commissioning has been completed. The Project Commissioning Report must include As-Built Diagrams of the commissioned ADG-to-Electricity and ADG Production Site systems. NYSERDA’s approval of the Project Commissioning Report will depend on the results of a post-installation site inspection, which will verify the information provided in the Project Commissioning Report. NYSERDA will provide notice of approval of the Project Commissioning Report or will request additional information within 60 days of receipt. Based on the information shown on the draft annual performance report, members of the CUB governing the program/ Service Enterprise will have an option to review and provide feedback on any corrective actions recommended by Enterprise Technology Services to address non-compliance with performance targets, as well as to monitor the implementation of the action plans agreed upon with Enterprise Technology Services throughout the previous twelve months for the measures that did not show compliance with the established performance targets. The reports will include a table (example provided below) showing the monthly kWh production, biogas use by the engine-generator, other data listed in Table 3, and if used, any propane or other fuel used for the subject engine-generator. The Annual Evaluation Report summarizes all data and the degree that outputs and outcomes are achieved. General Project Information Data to be Collected and Maintained KSU TAB will not collect environmental data. As part of this grant, KSU TAB will collect data, on a voluntary basis, on community members’ perceptions about the technical assistance provided, and if communities are meeting self-set goals.
