LEAVE AND LONG-TERM DISABILITY. (Articles to are related to Sick Leave and Long-term Disability will be incorporated in all collective agreements:) The Hospital shall provide a short-term sick leave plan at least equivalent to that described in the Hospitals of Ontario Disability Income Plan brochure. Copies of the brochure will be made available to employees upon request. The Hospital will pay seventy-five percent (75%) of the billed premium towards coverage of eligible employees under the long term disability plan or equivalent); employees shall pay the balance of the billed premiums through payroll deduction. The Hospital further agrees to pay employees an amount equal to any loss of benefits under for the first two days of the fourth and subsequent period of absence in any calendar year. Effective April employees with or more years service will be paid at the benefit level for all incidences of absence covered by Any dispute which may arise concerning an employee's entitlement to term or long-term benefits under may be subject to grievance and arbitration under the provisions of this Agreement. Payment will be provided only if the employee provides evidence of disability satisfactory to the Hospital and a written undertaking satisfactory to the Hospitalthat any paymentswill be refunded to the Hospital following final determination of the claim by The Workers' Compensation Board. If the claim for Workers' Compensation is not approved, the monies paid as an advance will be applied towards the benefits to which the employee would be entitled under the short term portion of the disability income plan. Effective January new hires will be covered under the Hospitals of Ontario Disability Income Plan. The Hospital will pay of the billed premium towards coverage of eligible employees under the long-term disability portion of the Plan or an equivalent plan). The employee will pay the balance of the billed premium through payroll deduction. For the purpose of transfer to the short-term portion of the disability program, employees on the payroll as of the effective date of the transfer with three (3) months or more of service shall be deemed to have three (3) months of service. For the purpose of transfer to the long-term portion of the disability program, employees on the active payroll as of the effective date of the transfer with one (1) year or more of service shall be deemed to have one (1) year of service. LEAVE AND LONG-TERM DISABILITY. The short-term sick plan will provide payment for the number of hours of absence according to the scheduled shift up to a fifteen (15) week total of hours. All other provisions of the existing plan shall be maintained. ARTICLE PAID HOLIDAYS (Applicable to Full-time Employees Only) Holiday pay will be computed on the basis of the employee’s regular straight time hourly rate of pay times the normal or standard work day as set out in the “Daily and Weekly Hours of Workprovision of the Local collective agreement (Article 16). An employee required to work on any of the designated holidays listed in the collective agreement shall be paid at the rate of time and one-half (1 his regular straight time rate of pay for all hours worked on such holiday (O001h to of the holiday). In addition, he will receive a lieu day off with pay in the amount of his regular straight time hourly rate of pay times seven and one-half (7 hours, except in those hospitals which have a different standard work day in which case the holiday pay will be based on the standard or normal daily hours in that hospital.
