Documentation of Sick Leave. The District may require documentation from a physician licensed to practice medicine, or a properly authorized physician assistant, as a basis for pay during sick leave after an absence of three (3) consecutive days for personal illness or injury, or as it may deem necessary in other cases.

Documentation of Sick Leave. When an employee utilizes sick leave for an absence of three (3) consecutive days or more, the employee may be required to furnish a certificate from the employee’s treating medical practitioner stating the nature of the illness, injury, treatment and prognosis. Additionally, if the Employer has reasonable cause to believe potential sick leave abuse is occurring, it may, in addition to any other remedy it may have, require an employee to submit a physician’s statement following any use of sick leave. Abuse of sick leave, falsification of an application for sick leave or a practitioner’s statement, or failure to submit adequate proof of illness or injury may result in the disapproval of sick leave, disciplinary action, or both.

AWOL is not a disciplinary action. It is a non-pay status that may form the basis for a disciplinary action. Medical certificates or other evidence of illness which may be required will be submitted within 15 days after the employee's return to duty. If, due to circumstances beyond the control of the employee, he or she is unable to provide the documentation within 15 calendar days, the employee must provide the evidence or medical certification no later than 30 calendar days after returning to work. An employee who does not provide the required evidence within 30 calendar days is not entitled to sick leave in accordance with 5 CFR 630.405(b). An employee can justify the request for sick leave by the following: By medical certification from the Department's personnel physician or the employee's personal physician or health care professional; By the employee's own written statement in instances where the illness was not treated by a physician. The statement will indicate why a physician was not seen, for example, remoteness of area, nature of illness, or other specific reasons. The supervisor may request clarification should the employee's written statement not be sufficient to support the request. An employee with chronic medical condition that does not require medical treatment, but does result in periodic absences from work, will not be required to furnish a physician's certificate on a continuing basis if the employee: Is not on leave restriction; and, Provides, if requested, an updated valid medical certificate every six months which clearly states the continuing need for periodic absences. Periods of absence on sick leave in excess of Three (3) consecutive workdays, or for a lesser period when management determines it is necessary, must ordinarily be supported by a medical certificate to be filed within fifteen (15) workdays after return to duty. In lieu of a medical certificate, the employee’s signed statement explaining the nature/purpose of her/his illness may be accepted, when it is unreasonable to require a medical certificate or because the illness does not require the services of a physician. Nature/purpose means whether or not the sick leave absence is because of illness/injury/incapacitation of the employee, care of a family member with a serious health condition or bereavement. For extenuating circumstances such as serious accident or illness, the supervisor will exercise due consideration of the circumstances in enforcing the reporting requirements.

Documentation of Sick Leave. Where medical verification is required, it is the employee’s responsibility to provide Human Resources with signed written statement, fully and accurately completed by an attending physician/medical practitioner. If the illness concerns the employee’s health, the signed written statement should document the employee’s related health condition and inability to work. If the employee verification concerns the health of an immediate family member who requires care from the employee, then the verification should be related to the family member’s health needs and the necessity for the employee’s presence. Employees are normally expected to document the use of sick leave within three (3) working days of the inception of their illness. The Employer recognizes that under certain rare circumstances, this may not be possible. However, no documentation will be accepted later than ten (10) working days after the inception of the employee’s absence.

  • Utilization of Sick Leave Employees who have accrued paid leave time shall be eligible for paid leave for any period of absence from employment which includes but is not limited to the employee’s illness; injury; temporary disability; medical or dental care; or to attend to members of the employee’s or the employee’s spouse’s immediate family or domestic partner or domestic partner’s immediate family, where the employee’s presence is required because of illness or as otherwise required by the state or federal Family Medical Leave Act or other State law. The Employer may, in its sole discretion, require reasonable proof of illness or disability and/or certification of the necessity of the employee’s absence.

  • Donation of Sick Leave The District shall allow unit members to donate sick leave to individual employees who have suffered long-term disabilities or illnesses. Part-time faculty may receive sick leave donations from any employee, and may donate accumulated sick leave to other part-time faculty following the same requirements as outlined above. Part-time faculty may not donate sick leave to full- time employees.

  • Abuse of Sick Leave Misuse of leave, violation of orders, directives, or contractual requirements concerning the use of sick leave and other forms of leave used in lieu of sick leave are cause for disciplinary action.
