Sick Leave Request. Teachers shall notify the appropriate administrator, with as much 31 advance notice as possible, when the use of sick leave is necessary. A claim for sick leave 32 on the proper form shall be signed by the teacher and filed with the principal or other 33 immediate supervisor by the end of the fifth working day following the employee’s return 34 to work.
Sick Leave Request. Whenever possible, teachers must submit their request for sick leave to their immediate supervisor in advance of the absence. When a teacher cannot obtain advance approval of his/her absence, it shall be his/her responsibility to notify his/her supervisor by telephone or other means as soon as possible, after his/her normal reporting time. Supervisors shall be required to answer all requests for sick leave promptly. Teachers using leave under this Section shall furnish a statement from a medical practitioner upon the request of the Appointing Authority for absences in excess of three (3) work days, or when the Appointing Authority has a reasonable basis to believe that a teacher has abused or is abusing sick leave. The Appointing Authority may also request a statement from a medical practitioner if the Appointing Authority has reason to believe the teacher is not physically fit to return to work or has been exposed to a contagious disease which endangers the health of other employees, clients or the public.

  • Sick Leave The employee is eligible for long term disability benefits if provided for in the Collective Agreement. An employee will not receive pay for the first two (2) weeks of any period of absence due to a legitimate illness. The employee may utilize the paid holiday bank as income replacement for absences due to illness, as described in Article (c) above. An employee who is eligible may apply for Employment Insurance for weeks three (3) through seventeen (17) for any absence due to a legitimate illness.   • Sick Leave Abuse When the Employer suspects sick leave abuse and notifies the employee, they will be given reasons for that suspicion and may be required to provide a written medical certificate for any sick leave absence. The Employer will not require continuous medical verification for longer than seven (7) months as a result of the Employer suspecting abuse. The Employer will not adopt or enforce any policy that counts the use of sick leave for an authorized purpose as an absence that may lead to or result in discipline. An authorized purpose is sick leave used in accordance with the terms and conditions of this Agreement and Agency Policy. The Employer will not discriminate or retaliate against an employee for the use of paid sick leave.

  • Extended Sick Leave When an employee’s salary continuation and short-term disability pay have been exhausted, the employee may request to be placed on extended sick leave if the employee continues to be unable to perform the duties of their position due to the illness or accident. Such request must be made thirty (30) days prior to the expiration of short-term disability payments. Extended sick leave may continue for up to eighteen (18) months from the employee’s last day worked. The time period may be extended by mutual agreement of the University and the POAM. If the employee is released to return to work during his/her extended sick leave, he/she will be offered the next available position for which he/she is qualified. Such position shall be equal in hours to the position previously held, unless otherwise agreed between the University, the employee, and the POAM. The University will pay the cost of COBRA medical coverage during the first twelve (12) months of extended sick leave.

  • Sick Leave Pool 1. Pool participants must have a minimum of six (6) days of accrued sick leave.

  • Sick Leave Records Immediately after the close of each calendar year, the Employer shall advise each employee in writing of the amount of sick leave accrued to his/her credit.
