Requesting Sick Leave. A. Employees must request and receive approval for use of sick leave from the designated or appropriate administrator in the Department as soon as possible after the onset of illness.

Requesting Sick Leave. Whenever practicable, employees shall submit written requests for sick leave, on forms furnished by the Employer, in advance of the period of absence. Supervisors shall respond promptly and shall answer all written requests in writing. When advance notice is not possible, employees shall notify their supervisor by telephone or other means at the earliest opportunity. Because of the personal nature of an illness, it is agreed that while supervisors must be informed of the general nature of an illness, such information shall be used only for administrative purposes. Departments shall post the names and phone numbers of supervisors designated to grant sick leave. Requests for immediate leaves (i.e., family sickness or death) shall be answered before the end of the shift on which the request is submitted. The Union and the Employer agree that sick leave is a benefit to be used judiciously and should not be abused.
Requesting Sick Leave. Requests for use of sick leave shall be signed by the employee and subject to the approval of the department head. Requests to use sick leave credits for a death in the employee's immediate family shall be in accordance with the Funeral Leave section of the Leaves of Absence Article.

Requesting Sick Leave. Except in emergency circumstances, employees should request sick leave in advance. The employee is responsible for notifying his/her supervisor or designee when he/she is prevented from reporting to work because of illness or injury. When requests for sick leave are in excess of three (3) workdays, the supervisor may also require a medical certificate or other evidence as to the reason for an absence. Leave approving officials have the responsibility to determine that the circumstance of the absence justifies approval of the sick leave request.

When an employee is unable to request sick leave in advance because of unforeseen sickness, injury or other circumstances, the employee, if possible, shall make a reasonable effort to inform his or her immediate supervisor of the need for sick leave and the expected return-to-duty date by telephone no later than the beginning of the employee’s fixed schedule or the core period. If the supervisor is unavailable, the employee must use the call transfer procedure to talk to or leave a message with the person to whom the call is forwarded. The employee may be transferred to another supervisor by that person. Employees may be required for good and sufficient reasons to call in to report continued sickness or incapacitation on each day they are absent unless sick leave for a continued sickness or incapacitation has been previously approved.

Medical documentation is confidential and will only be shared on a need to know basis. Leave approving officials will request medical documentation in an equitable manner, as appropriate for the circumstances.  Employees will not normally be required to furnish medical documentation to substantiate a request for approval of sick leave for sick leave periods of three consecutive workdays or less unless the Employer has given written notice to the employee that he or she is under a leave restriction for a stated period (generally six months), or there are specific circumstances that suggest the employee may be misusing sick leave.
