Medical Verification definition

Medical Verification means documentation required by the department provided by a physician to a patient in the course of a bona fide physician‑patient relationship for the patient’s submission to the department with an application for a registry identification card, which supports the physician’s opinion that the patient has a debilitating medical condition with symptoms or side effects that might be alleviated by the medical use of marijuana and that reasonable medical efforts have been made over a reasonable amount of time without success to relieve the symptoms or side‑effects.
Medical Verification. AB2007 requires that the District determine that the Officer is unable to work due to a catastrophic illness or injury. Requests: AB2007 specifies that an Officer who is suffering from a catastrophic illness or injury must request that eligible vacation or sick leave is donated. Donations cannot be accepted or approved without this request. FAMILY AND MEDICAL LEAVE ACT (FMLA)
Medical Verification. The health associate shall be required to provide the School District with medical verification from a qualified healthcare provider for their own or the family member’s serious health condition when requesting the leave of absence.

  • A "terminal illness" must be certified as such by the procedure established in Article 11A.5 -Medical Verification.

  • The “Request for Medical Verification of Health Status and Needs Form” shall have the appropriate procedures to be utilized as approved by the physician attached to the completed form.

  • A Medical Verification form must be submitted with the Request for Leave form.

  • In such event, the faculty member should submit the Medical Verification Form (on FSCJ HR website) to Human Resources.
