Health Care Professional. A physician or any of the following: podiatrist, optometrist, chiropractor, psychologist, dentist, Physician Assistant, physical or occupational therapist, therapist assistant, speech-language pathologist, audiologist, Registered or practical Nurse (including nurse practitioner, clinical nurse specialist, certified Registered Nurse anesthetist and certified nurse midwife), a licensed certified social worker, registered respiratory therapist and certified respiratory therapy technician.
Health Care Professional. Health Insurance Portability and Accountability Act of 1996 (HIPAA) – The Health Insurance Portability and Accountability Act of 1996 and the regulations issued pursuant thereto or as thereafter amended. Individual Exchange –Covered California through which Qualified Individuals may purchase QDPs. Individually Identifiable Health Information (IIHI) – The “individually identifiable health information” as defined under HIPAA. Information Practices Act (IPA) – The California Information Practices Act, Civil Code § 1798 et seq. and the regulations issued pursuant thereto or as thereafter amended. Insurance Information and Privacy Protection Act (IIPPA) – The California Insurance Information and Privacy Protection Act, Insurance Code §§ 791-791.28 et seq., and the regulations issued pursuant thereto or as thereafter amended.

Health Care Professional. The Healthcare Professional must also be either licensed or practice under the authority of a licensed Health Care Professional.
Health Care Professional. By partaking in the Food & Spirit Training Program you are agreeing that you are a health care professional and that you will represent yourself as such while using any Food & Spirit modalities, in accordance with your state laws and governing organization. If at any time you misrepresent yourself as a health care professional or cease to be a healthcare professional, then you will lose your certification immediately.

Date of admission and dates of application for and authorization of Medicaid Benefits if application is made after admission; the plan of care (as required under 456.172 (mental hospitals) or 456.70 (hospitals).
