Purpose of Request. FOR PROPOSAL (RFP) The State of Utah, Division of Purchasing & General Services (Lead State) is requesting proposals for office furniture and related accessories including all customer service, installation, and design services in furtherance of the NASPO ValuePoint Cooperative Purchasing Program. The purpose of this Request for Proposals (RFP) is to establish Master Agreements with qualified original equipment manufacturers (OEM’s) and their Authorized Dealers to provide office furniture products and related services for all Participating States. Authorized Dealers are required to offer customer service, sales service, warranty service, design services, and installation services. This solicitation does not allow for multiple firms represented by a distributor to respond. This solicitation does not allow for sister companies to team and submit offers. The objective of this RFP is to obtain best value, and in some cases achieve more favorable pricing, than is obtainable by an individual state or local government entity because of the collective volume of potential purchases by numerous state and local government entities. The Master Agreement(s) resulting from this procurement may be used by state governments (including departments, agencies, institutions), institutions of higher education, political subdivisions (i.e., colleges, school districts, counties, cities, etc.), the District of Columbia, territories of the United States, and other eligible entities subject to approval of the individual state procurement director and compliance with local statutory and regulatory provisions. The term of the Master Agreement shall be five (5) years. Any resulting Participating Addenda for a State of Utah Cooperative Contract will be for the benefit of all Utah public entities, nonprofit organizations, and agencies of the federal government, i.e. State of Utah departments, agencies, and institutions, political subdivisions (colleges, universities, school districts, special service districts, cities and counties, etc.). The State of Utah Division of Purchasing does not guarantee any purchase amount under an awarded contract. Estimated quantities are for solicitation purposes only and are not to be construed as a guarantee. It is anticipated that this RFP may result in Master Agreement awards to multiple contractors. Purpose of Request. It is the intent of the NGS Remote Sensing Division to collect high-resolution shoreline data of the port of Detroit using airborne imagery equipment.
Purpose of Request. The original Development Agreement with South Bend Heritage Foundation (SBHF) regarding the Gemini at Colfax Apartments stipulated that $1M in City funding would be allocated to the property in exchange for a maximum for six (6) Permanent Supportive Housing vouchers being used at the property to help with the City’s housing issues. The original agreement had a completion date of December 31, 2021. The City successfully completed four (4) projects at the property: • Roof Replacement: $143,679 • Masonry Repairs: $368,128 • New Fire Stairs: $163,000 • New Water Line: $65,457 The City attempted a fifth project covering window replacement ($207,000), but the project stalled out during the COVID pandemic in 2021, leading to a contract cancellation through Board of Public Works in April 2022. SBHF maintains possession of the windows purchased by the City, and will work with a local contractor to install the remaining windows at their own labor cost. $70,000 remains from the original project budget after completion of the four projects mentioned above, as well as the contract cancellation regarding the windows. The City has been working with SBHF to scope out a small project to use the remaining funds. SBHF has a small masonry and sidewalk project scoped out to address deteriorating areas. Purpose of Request. Data User has requested MHA to disclose to Data User data from a Data Set to be used by Data User consistent with this Agreement for the purpose(s) stated in an attached Addendum. Additional Addenda may be attached to this Agreement for multiple data requests by Data User. For any such request, Data User warrants that Data User's request is limited to the minimum necessary data to accomplish the intended purpose of the request and all further uses and disclosures of the Data Set by Data User must be necessary to carry out the stated purpose. Data User represents and warrants to MHA that the purpose(s) stated in the attached Addendum qualifies as research, public health or "health care operations" of a named "covered entity" as those terms are defined in HIPAA. Data User shall indemnify, defend and hold MHA harmless from all claims, liability, causes of action and damages (including civil monetary penalties or settlement amounts entered into with the Secretary of HHS or the Office of Civil Rights) arising out of or in any way relating to the purpose for which the Data Sets were disclosed to Data User or relating to MHA's reliance on the representations provided herein by Data User.

Purpose of Request. On behalf of Reems Ranch, LLC (“Reems Ranch”), we request a Military Compatibility Permit (“MCP”) without a precise Plan of Development for approximately 260 acres located south of the SEC of Reems Road and Northern Avenue (the “Site”). A Regional Aerial/Vicinity Map generally identifying the perimeters of the Site is provided at Exhibit 1. The Site is bounded by Reems Road to the west, Luke Air Force Base (“Luke AFB”) to the east, and an approximate 80-acre privately owned parcel to the north that is currently leased by Reems Ranch for agricultural purposes. As a follow-up to the approval of the Use Compatibility and Consistency Determination for Reems Ranch (“UCCD” - see Appendix A), case #UC2018005, the purpose of this request is to obtain approval for the development of a new industrial facility (or similar type of development) that is consistent and compatible with the high noise or accident potential of the Luke AFB military facility. A Conceptual Land Use Exhibit is provided at Exhibit 2. The Site is within Maricopa County’s White Tanks/Grand Avenue Area Plan and the City of Glendale’s Planning Area. The proposed land uses comply with the White Tanks/Grand Avenue Area Plan designation of “Military Compatible” and the City of Glendale General Plan’s designation of “Luke Compatible,” as both require uses compatible with the adjacent Luke AFB operations. Such compatibility was confirmed in the Site’s approved UCCD. The Site is zoned RU-43 with Military Airport and Ancillary Military Facility Overlay Zoning. Reems Ranch’s proposed land uses (see Table 1 below) are compatible with the Military Airport and Ancillary Military Facility Overlay Zoning and the operations of Luke AFB. The Military Airport and Ancillary Military Facility Overlay requires a Military Compatibility Permit to allow the proposed land uses.
