UNDERSTANDING AND AGREEMENT. I understand that every fellow member of MHN-­‐PMA, who provides services and care, does so in the capacity of a fellow member and not in the capacity of a licensed health care provider. Any request by me to a fellow MHN-­‐PMA member to assist in natural health modalities is an exercise of my rights and free will and made by me for my benefit, and I agree to hold all MHN-­‐PMA staff and worker members and the MHN-­‐PMA at large harmless from any unintentional liability for the results of such care, except for harm that results from instances of clear and present danger of substantive evil as stated and defined by the Supreme Court of the United States. I understand that the MHN-­‐PMA members have chosen Xxxxxx Xxxxxxxxxxx, ND as the member best qualified to perform services for MHN-­‐PMA members and entrust to her the selection of other MHN-­‐PMA members to assist her in carrying out those services. I further understand that since the First and Fourteenth Amendments to the U.S. Constitution protect the MHN-­‐PMA, it is outside the jurisdiction and authority of federal and state agencies and authorities concerning any and all complaints or grievances against MHN-­‐PMA, including any member or other staff person. An MHN-­‐PMA member committee will settle all complaints or grievances. I waive any other recourse I might have in such matters. Because the privacy and security of membership records maintained within MHA-­‐PMA have been held to be inviolate by the U.S. Supreme Court, I waive my HIPPA privacy and any and all associated complaint processes. Any medical or health records that are kept on file for an MHN-­‐PMA member will be strictly protected and only released by written request of the affected member.
