Health Professionals. The expenses indicated below are reimbursed as per the terms and conditions set out in the Schedule of Benefits and based on the chosen module. All of the health professionals mentioned in this document must be members in good standing with their professional corporation or association or be recognized by the Policyholder and the Insurer and must practice within their competence, as defined by law. For each of the health professionals listed below, only one session of care or treatment per day for the same patient is eligible for benefits:

Health Professionals. A Physician, dentist, podiatrist, pharmacist, optometrist, psychologist, clinical social worker, advance practice nurse, registered optician, licensed professional counselor, physical therapist, marriage and family therapist, chiropractor, athletic trainer, occupational therapist, speech language pathologist, audiologist, dietitian, or physician's assistant or other professional who is properly licensed or certified to provide medical care under the laws of the state where the individual practices, and who provides Medical Services which are within the scope of the individual’s license or certificate. • Health Maintenance Organization (HMO). Aetna Health Inc., a Georgia corporation licensed by the Georgia Department of Insurance as a Health Maintenance Organization. • Homebound Member. A Member who is confined to their place of residence due to an illness or injury which makes leaving the home medically contraindicated or if the act of transport would be a serious risk to their life or health. Examples where a Member would not be considered homebound are:

Assist in the development of policies, procedures, standards, and practices, participate in quality improvement activities, and may participate in clinical research activities as required. HP – 3 Positions that are typically classified at this level include, but are not limited to: Senior level Physiotherapist Occupational Therapist Bachelor’s Degree in relevant discipline plus experience. Health Professionals. All health professional positions are non-tenure bearing positions which are governed by applicable provisions of the Civil Service Law. All nurses, COTAs, physical therapists and occupational therapists are to work in accordance with the job description in Appendix F.
