Health Programs. 109. State Parties shall operate and maintain funding at current levels for the programs listed below, until the dates listed below, subject to applicable federal laws, necessary approvals from the Centers for Medicare & Medicaid Services, and availability of necessary federal monies:
a. The Medicaid expansion for pregnant women and children under twenty-one years of age up to 400% of the federal poverty level (as defined by the U.S. Department of Health and Human Services), through March 2021;
b. Elevated Blood Lead Level Case Management services for children with elevated blood lead levels, through September 2018;
c. Family Supports Coordination target case management services, through March 2021;
d. The Michigan Child Collaborate Care expansion, through September 2018;
e. The Child and Adolescent Health Center expansion, through September 2018;
f. The Michigan Nurse Professional Fund, through September 2018;
g. The Mobile Food Pantry expansion, through September 2018;
h. The Double Up Food Bucks expansion, through September 2018; and
i. The Summer Electronic Benefit Transfer for Children program expansion, through September 2018.
Health Programs. Immunization - a nurse, upon request, may receive immunization at the Hospital's expense.
Health Programs. Educational Support Series employees who are in the bargaining unit shall be covered by the same health insurance benefits as specified in Article 20.
Health Programs. A. The Township shall maintain all present State Health Benefit Plan hospital and medical insurance programs to which Patrol Officers are presently beneficiaries, including dependent coverage. The cost of coverage for the employee and dependents will be in accordance with state law. The Township’s base plan is NJDIRECT15. If a Patrol Officer elects coverage other than NJDIRECT15, he/she shall pay all the costs above the cost of NJDIRECT15 associated with the coverage elected. The maximum for chiropractic services shall be One Thousand Dollars ($1,000.00).
B. The Township shall continually investigate a more comprehensive group hospital and medical insurance program and, if such a program is adopted for other Township employees, it shall automatically apply to Township Patrol Officers, provided substantially equivalent benefits and coverages are contained.
Health Programs. The Sheriff will comply with Public Act 57 of 1997 (MCL333.5204) HIV HBV Testing, Tuberculin Testing and Influenza Immunizations.
Health Programs. 18.01 The District will make available health coverage under the CalPERS Health Benefits Program. Enrollment in the CalPERS Health Plans is mandatory with the exception of the provisions of Dual Coverage benefit described in paragraph b below. The same plan will cover both the employee and his/her eligible dependents pursuant to CalPERS Health Benefits Program eligibility requirements. The District has implemented a full Flex Cafeteria Plan, effective January 1, 2009, hereby referred to as the “Cafeteria Plan”, for all eligible employees. Health Programs. Immunization, where available through existing Public Health Service facilities, shall be provided free to employees. Employee health examinations shall be offered to unit employees within the allocation, if any, made available by Management within that office. Selections for examinations shall be made from among the unit employees at a given office on a fair and equitable basis. Employee participation shall be voluntary. Management shall advise the Union in advance of such examinations so that eligible employees may apply. Management shall propose to the Union the criteria to be used for selection and such criteria may be the subject of supplemental bargaining.
Health Programs. The Hospital agrees to continue three medical plans for benefit year 2019, the HSA, Value and PPO plans. The Hospital agrees that current deductibles, wellness incentives, out of pocket maximums and amount of the Employer HSA contribution will stay the same. The Hospital also agrees that one-half of the Employer’s HSA contribution will be front loaded in January of 2019.
