SERVICE/PRODUCT DESCRIPTION. The Service is provided by AT&T Corp., acting on it’s own and behalf of it’s affiliated entities. Service is offered via supplier’s facilities in combination with resold exchange service, intralata services, or transmission facilities provided by other certified carriers. Service is only available to the connection of individual points of presence for carriers PrimeXpresssm. AT&T PrimeXpress is a premier digital trunking service that provides users with PBX or PBX-like equipment T1 access to the ALS network for voice access. AT&T PrimeXpress offers the flexibility of providing outbound, inbound, or two-way service with 24 voice paths per facility. PrimeXpress Network Service is offered with the options of DTMF or MF signaling pulse and wink start or Immediate start trunk signaling. PrimeXpress provides users with T1 access with 24 voice paths per facility to the ALS Class 5 switching center. PrimeXpress also offers a complete assortment of calling services and calling plans, including competitively priced local and regional toll calling plans. PrimeXpress also allows for the long distance carrier of the State’s choice. AT&T PrimeXpress supports the following trunk types: ♦ DOD - Allows station users behind a PBX to dial directly out from their extensions without the assistance of an attendant. The service is one-way only and is a basic trunk. ♦ Combo - Two-way trunk group which allows DOD dialing from station users behind a PBX and incoming calls to an attendant. The service does not allow direct call completion to a station user behind a PBX. ♦ DID - Allows incoming calls to be terminated directly to an extension behind a PBX without the need for an attendant to complete the call. The service is one-way only and supports a maximum of 500 DID numbers per T1. ♦ DID/DOD - Provides two-way trunking which allows DID and DOD calls to be placed on each channel within the group. SERVICE/PRODUCT DESCRIPTION. Managed Internet Service (MIS) is an Internet access service that combines a high-speed, dedicated connection with consolidated application management. It lets you reliably access information resources and communicate with Internet users worldwide. MIS includes proactive, 24x7 network monitoring, enhanced network security features, and maintenance of the communications link between your locations and the AT&T network. MIS is available in two service types. MIS with Managed Router provides end-to-end managed Internet access. AT&T configures, install, and manage your on-site router, Channel Service Unit/Data Service Unit (CSU/DSU), and diagnostic modem. MIS with Customer-Managed Router provides managed Internet access but allows you to provide and manage your own on-site equipment.   • GENERAL SERVICE DESCRIPTION Service Provider currently provides active medical, pharmacy(Rx) and dental administration for coverages provided through Empire and Anthem (medical), Medco(Rx), MetLife(dental) and SHPS (FSA) (Empire, Anthem, Medco, MetLife and SHPS collectively, the “Vendors”) for its U.S. Active, Salaried, Eligible Employees (“Covered Employees”). Service Provider shall keep the current contracts with the Vendors and the ITT CORPORATION SALARIED MEDICAL AND DENTAL PLAN (PLAN NUMBER 502 EIN 00-0000000) and the ITT Salaried Medical Plan and Salaried Dental Plan General Plan Terms (collectively, the “Plans”) and all coverage thereunder in full force through December 31, 2011 for Service Recipient’s Covered Employees. All claims of Service Recipient’s Covered Employees made under the Plans and incurred on or prior to December 31, 2011 the (“2011 Plan Year”) will be adjudicated in accordance with the current contract and Service Provider will continue to take such actions on behalf of Service Recipient’s Covered Employees as if such employees are employees of Service Provider. All medical, dental, pharmacy and FSA claims of Service Recipient’s Covered Employees made under the Plans (the “Claims”) will be paid by the Vendors on behalf of the Service Provider. Service Recipient will pay Service Provider for coverage based on 2011 budget premium rates previously set for the calendar year 2011 and described in the “Pricing” section below. Service Recipient will pay Service Provider monthly premium payments for this service, for any full or partial months, based on actual enrollment for the months covered post-spin using enrollments as of the first (1st) calendar day of the month, commencing on the day after the Distribution Date.   • System Description The wet detention basin is designed to trap 80% of sediment in runoff and maintain pre-development downstream peak flows. The basin has two forebays (smaller ponds) located at the low end of two grass xxxxxx. In addition to runoff conveyance, the grass xxxxxx also allow infiltration and filtering of pollutants, especially from smaller storms. The forebays are each 4 feet deep. They are connected to the main pool by 18 and 24-inch metal pipes that outlet onto a rock chute. The forebays will trap coarse sediments in runoff, such as road sands, thus reducing maintenance of the main basin. The main pool will trap the finer suspended sediment. To do this, the pond size, water level and outlet structures must be maintained as specified in this Agreement (see Figures 1, 2 and 3). The main basin receives runoff from a 67.1 acre drainage area (41.2 acres within the subdivision and 25.9 acres off-site drainage coming from the east). During high rainfall or snow melt events, the water level will temporarily rise and slowly drain down to the elevation of the control structure. The water level is controlled by a 12-inch concrete pipe extending through the berm in the northwest corner of the basin (see Figures 1 and 3). On the face of the 12-inch pipe, there is metal plate with a 3-inch drilled hole (orifice) with stone in front of it. This orifice controls the water level and causes the pond to temporarily rise during runoff events. Washed stone (1- 2” diameter) is placed in front of the orifice to prevent clogging. High flows may enter the grated concrete riser or flow over the rock lined emergency spillway. “As-built” construction drawings of the basin, showing actual dimensions, elevations, outlet structures, etc. will be recorded as an addendum(s) to this agreement within 60 days after [Municipality Name] accepts verification of construction from the project engineer.

No costs or expenses associated with providing this information in the required format shall be charged to the State of Arkansas. At the time of contract renewal contractor will furnish OSP with an updated dealer list and published price list.
