Health Insurance Continuation. You and your beneficiaries, if applicable, will receive under separate cover notice of your rights under the Consolidated Omnibus Reconciliation Act (“COBRA”) upon your separation from the Company. If you elect to continue health care coverage under COBRA, you are responsible for paying the premiums.

Health Insurance Continuation. Employees “laid off” by the City shall continue to receive the above-described health care coverage benefits for the period of six (6) months following the day of “layoff” for involuntary termination of employment; except where the employee was “fired for cause,” in which case the coverage benefits shall be terminated and cease as soon as possible under the City’s policy.
Health Insurance Continuation. Each of the participants in the Program may continue to participate in the District’s group health insurance plan (continue the same individual or family coverage elections as applied prior to retirement) following retirement for twenty-four (24) months. The District shall contribute to the retired teacher’s premium for the medical insurance portion (but not vision and dental) of such plan on the same basis (with exceptions noted below) as a regularly employed teacher for a period of twenty-four (24) months following the cessation of regular health insurance coverage on July 31 of each year. Retiring employees who work a transition year receive insurance benefits, with the exception of Long- Term Disability insurance, in connection with the twelve (12) month transition year employment. The employee will then be eligible for continued health insurance benefits for the remaining twelve (12) months of the Article XVII Retirement Incentive Program benefits. COBRA benefits will only be available if the COBRA benefit period (normally 18 months) extends beyond the other benefits coverage periods, as COBRA coverage periods run concurrently with other benefits provided after termination of employment, including termination of the transition year, if applicable. Medicare eligible retirees will receive medical insurance benefits assuming enrollment in Medicare Part A and Part B.
Health Insurance Continuation. Employees who retire and do not meet the severance eligibility criteria in Section 1. and are enrolled in the district group health and hospitalization plan prior to retirement may elect to continue participation in the plan, at the employee’s expense, until the employee reaches the age of Medicare eligibility if they are receiving a disability benefit or an annuity from a MN public pension plan as outlined in Minnesota Statute § 471.61.

Health Insurance Continuation. Lake Superior State University offers the opportunity for retired staff members and their spouses to continue health, dental and vision insurance benefits as they may be made available by the University. Availability of health insurance benefits to retirees and their spouses is subject to the concurrence of the benefit carriers. An official University retiree is defined as one who has completed ten (10) years of continuous full-time or equivalent part-time service at Lake Superior State University and has met the age and service requirements for retirement as defined under the provisions of the Michigan Public School Employees Retirement System MIP program. Retirees may participate in the available benefit program only if they are not being provided with a health benefit insurance paid, in whole or in part, by another entity. Application for continued health insurance must be made at the University’s Human Resources Office no less than 30 days prior to the effective date of retirement. The retiree is fully responsible for the payment of all premiums in order to maintain coverage. The University will issue guidelines for receipt of payment. Failure to meet those guidelines will mean discontinuance of coverage. If the retiree precedes a spouse in death, the spouse may continue participation in the health insurance program the University may offer for up to the period of time COBRA law allows (currently 36 months). Health Insurance Continuation. 17.6.1. The College will continue to pay its share of all group insurance benefits consistent with the procedure set forth in this Agreement during an unpaid leave of absence such that said group coverages are continued through the end of the calendar month in which the employee last actually worked, or was on paid leave status, whichever is later. Beginning with the first day of the calendar month immediately following the month in which the employee last actually worked or was on paid leave status, whichever is later, the employee shall be responsible for securing insurance benefits and such will be provided by the College consistent with its obligations under COBRA.

Health Insurance Continuation. Pursuant to the provisions of the Internal Revenue Code of 1986, as amended, and the Employee Retirement Income Security Act of 1974, as amended, commonly referred to as “COBRA,” Laclede will provide the required COBRA notification within fourteen (14) days of the Separation Date and the COBRA benefit entitlement period of eighteen (18) months shall commence to run effective on October 1, 2007. Xxxxxx’x existing insurance coverage will expire at the end of the day on September 30, 2007. Xxxxxx and any of his eligible dependents, as applicable, may elect COBRA coverage under the provisions of COBRA on October 1, 2007. Should Xxxxxx or any of his eligible dependents elect COBRA coverage, Xxxxxx will be solely responsible for the full cost of premiums associated with any such election.
Health Insurance Continuation. If you presently have insurance through the Company plan, your group health insurance will cease on the last day of the month in which your employment ended. At that time, you will be eligible to continue your group health insurance benefits at your own expense, except as described below, subject to the terms and conditions of the benefit plan, federal COBRA law, and, as applicable, state insurance laws, and you will be responsible for COBRA enrollment. You will receive additional information regarding your right to elect continued coverage under COBRA in a separate communication. The Company does not take responsibility of enrolling or making COBRA payments on behalf of the employee, the process of COBRA election and payment is the responsibility of the Employee.
Health Insurance Continuation. In the event benefits are paid under this Article, Executive shall be eligible for the benefits provided for in section 3.3(b).
