Health/Hospitalization and Dental Insurance a. Full-Time: A teacher employed 0.8 to 1.0 FTE will be eligible for full-time benefits under the terms of the District Health/Hospitalization and Dental Insurance Plans. This includes CE teachers scheduled to work no less than thirty (30) hours per week and one thousand ninety-eight (1,098) hours per year.

Health/Hospitalization and Dental Insurance. The School District shall offer the Superintendent single and family health/hospitalization and dental insurance, as provided under the School District’s group plans. If the Superintendent qualifies for and is enrolled in a School District group plan(s), the School District shall pay the full cost of the premiums for the 2012-2013 Contract year (July 1 through June 30). If the cost of the insurance premiums increase after the first Contract year, the School District will increase its contributions to the premiums in an amount not to exceed 5% in each Contract year. To the extent the cost of insurance premiums exceeds the School District’s contributions, the Superintendent shall be responsible to pay the cost of the monthly premiums through payroll deduction. If the Superintendent participates in the School District’s group dental insurance plan, the School District shall pay the full cost of the premiums for the first Contract year (July 1 through June 30). If the cost of premiums increase after the first Contract year, the School District will increase its contributions to the premiums in an amount not to exceed 5% each Contract year. To the extent the cost of dental insurance premiums exceeds the School District contributions, the Superintendent shall be responsible to pay the cost of the monthly premiums through payroll deduction.
Health/Hospitalization and Dental Insurance. The School District shall contract with an insurance carrier(s) to provide health, hospitalization, and dental insurance for each teacher and his/her eligible dependents as long as those teachers qualify for and are enrolled in the School District’s group health and hospitalization plan. The selection of the insurance carrier and policy shall be made by the School District as provided by law.

The City shall pay 78% of the actual cost of a premium toward the cost of a single plus spouse, single plus child or children or family policy per month for employees but not to exceed $1,329.69 with respect to a single plus spouse plan or $1,208.80 with respect to a single plus child or children plan or $1,558.73 with respect to a single plus family plan, in addition to the monthly contribution toward dental insurance established in Section 2 herein; provided, further, that if the employee elects coverage under a PPO or indemnity option, the City’s contribution to payment for such PPO or indemnity option shall be in the same dollar amount as if the employee had elected coverage under the HMO option at the same tier level. For calendar year 2019 and subject to and conditioned upon the Union agreeing to health benefit plan(s) specifications recommended and approved by the City which are less than those in effect on December 31, 2003, the cap amounts set forth in Section 25.1 shall be increased by fifty percent (50%) of any increase in the actual cost of the 2018 monthly premiums and the 2019 monthly premiums. By way of example, if the 2018 monthly premium for a single plus family plan increases by $30.00 in 2019, the cap amount for calendar year 2019 will be $1,573.73 or $1,558.73 + $15.00; or, alternatively, if the specifications recommended and approved by the City result in no increase in premiums, there shall be no increase in the cap amount.
Health/Hospitalization and Dental Insurance. The District shall provide dependent coverage for each principal employed by the District who qualifies for and is enrolled in the School District Employee Group Hospitalization Plan and Group Dental Plan. In the event this contract will cause or does cause penalties, fees or fines to be assessed against the School District, the parties agree to reopen negotiations that result in a revised contract between the parties that eliminates or reduces penalties, fees or fines to be assessed against the School District. The amount of any reduction in the School District’s contribution toward the Principal’s healthcare benefits as a result of addressing the “highly compensated employee” component of the ACA will be placed into another School District provided benefit(s) (i.e. a retirement HRA, salary, etc.) as agreed upon between the parties. For Principal’s hired after July 1, 2021, the School District shall provide each principal with a health insurance policy under the School District’s plan, at the expense of the School District not to exceed $22,706 for a family plan and $8,661 for a single plan. If the principal chooses to take a high deductible plan, the district contribution to the plan would be the full cost of the plan and the remaining amount into an HSA/VEBA. The cost of the contribution to the plan and the contribution to the HSA/VEBA together will not exceed $22,706 for family and $8,661 for single. The District shall provide dependent coverage for each principal employed by the District who qualifies for and is enrolled in the School District Employee Group Dental plan. In the event this Contract will cause or does cause penalties, fees, or fines to be assessed against the School District, the parties agree to reopen negotiations that result in a revised Contract between the parties that eliminates or reduces penalties, fees, or fines to be assessed against the School District. Health insurance shall be as provided or otherwise determined pursuant to the August 26, 2019 Self-Funded Health Benefit Plan Amendment to the Labor Agreement, a copy of which is attached hereto and labelled Appendix E; provided, however, subsection 1.b. shall be amended to read as follows: Premiums and Contributions. Premiums shall be determined by City based upon the recommendation of City’s insurance broker consultant, the plan design chosen pursuant to above Subsection 1.a., and the funding level acceptable to City. Employees who are eligible and select an offered plan shall pay the cost of any applicable premium by payroll deduction subject to the contributions herein provided. The City shall pay 95% of the actual premium for an individual policy per month but not to exceed $899.62. The City shall pay 78% of the actual cost of a premium toward the cost of a single plus spouse, single plus child or children or family policy per month for employees but not to exceed $1,563.15 with respect to a single plus spouse plan or $1,423.31 with respect to a single plus child or children plan or $1,828.11 with respect to a single plus family plan (“Contribution Cap”). The Contribution Cap applicable to each coverage tier, i.e. individual, single plus spouse, single plus child or children or family coverage tier, shall be increased by fifty percent (50%) of any increase between the HMO Tier 1 premium for such coverage tier in the current year and the recommended HMO Tier 1 premium for such coverage tier in the subsequent year. If the employee elects coverage under a PPO option, if offered, the City’s contribution to payment for such PPO shall be in the same dollar amount as if the employee had elected coverage under the HMO Tier 1 option for the same coverage tier. If an employee elects coverage under a high deductible health plan with Health Savings Account (“High Deductible HSA”), if offered, the City’s monthly contribution for the High Deductible HSA shall be One Hundred Dollars ($100.00) per month less than monthly contribution required for the City’s HMO Tier 1 plan, at the same coverage tier, and the City will contribute Sixty Six Dollars and 67 Cents ($66.67) per month to each employee’s health savings account under such High Deductible HSA.

VEBA (vision and dental only) $1500 annual contribution *any teacher with continuing health coverage outside of Pine Island Schools may elect to participate in a VEBA Plan. Upon doing so, they will annually have $1,500 deposited in nine (9) equal installments into the designated account with the School District’s VEBA Health Plan (Vision/Dental only) established in September 2013. Teachers can opt back into the School District’s health and hospitalization plan subject to insurance carrier's provisions (open enrollment or “life” event). Additional benefits and clarification: *Married employee couples will get the family contribution plus the single contribution toward a family policy premium. *Married employee couples will get the employee plus one contribution plus the single contribution toward an employee plus one policy premium. *In the event that premiums are less than the district contribution, the district will contribute dollars to an HSA to reach the district maximum contribution. .

Effective September 1, 2019, and September 1, 2020, the School District shall contribute a sum not to exceed $700 and $720, respectively per month toward the premium for coverage for each full- time employee employed by the School District who qualifies for and is enrolled in the School District group health and hospitalization plan. The School District shall provide single coverage for the District dental plan for all full-time employees employed by the School District. The School District contribution shall not exceed 100% of the single premium per month effective September 1, 2019 and September 1, 2020.
