Health Plan. An appropriately licensed entity that has entered into a contract with Subcontractor, either directly or indirectly, under which Subcontractor provides certain administrative services for Health Plan pursuant to the State Contract. For purposes of this Appendix, Health Plan refers to UnitedHealthcare Insurance Company.
Health Plan. The City shall provide a group comprehensive health care plan to regular employees. The City shall pay one hundred percent (100%) of the monthly cost of single coverage. The City will pay ninety-four percent (94%) of the monthly cost of family coverage and ninety-four percent (94%) of the monthly cost of 2/4 party coverage.
Health Plan. 5 A plan that undertakes to arrange for the provision of health care services to subscribers or 6 enrollees, or to pay for or to reimburse for any part of the cost for those services, in return for 7 a prepaid or periodic charge paid for by or on behalf of subscribers or enrollees.
Health Plan. (a) Producers, the IATSE, and the respective West Coast Locals are parties to the "Agreement and Declaration of Trust establishing the Health Plan for the Employees of the Motion Picture Industry" (hereinafter referred to as the "Health Plan") made as of October 20, 1952. Such Health Plan was established in accordance with the provisions of the "Producer-I.A.T.S.E. and M.P.M.O. Supplemental Agreement of October 25, 1951."
Health Plan. The District will provide group health benefits through the California Public Employees’ Retirement System (CalPERS) for all permanent full-time employees and permanent part-time employees regularly scheduled to work at least twenty (20) hours per week in classes represented by the IAFF, Local 1230. The CalPERS health care program, as regulated by the Public Employees’ Medical and Hospital Care Act (PEMHCA), regulations issued pursuant to PEMHCA, and the administration of PEMHCA by CalPERS, controls on all health plan issues for employees who receive health care coverage from CalPERS, including, but not limited to, eligibility, benefit plans, benefit levels, minimum premium subsidies, and costs.
Health Plan. The State shall provide health plan coverage for employees pursuant to Title 5 §285. The State shall pay sixty percent (60%) of the cost of dependent premium for each eligible employee who selects dependent coverage.
Health Plan. 16 A plan that undertakes to arrange for the provision of health care services to subscribers or 17 enrollees, or to pay for or to reimburse for any part of the cost for those services, in return for 18 a prepaid or periodic charge paid for by or on behalf of subscribers or enrollees.
Health Plan. The Employer agrees during the duration of this contract to pay one hundred percent (100%) of the cost of the monthly premium of the Health Plan for employees covered thereby.
Health Plan. The Department will contribute on behalf of eligible employees in this Unit, to whom an annual salary rate is applicable, and who are members of the Water and Power Employees’ Retirement, Disability and Death Benefit Plan, and on behalf of their eligible dependents, if any, a sum not to exceed $823.76 a month, through June 30, 2005, toward the cost of any one of the following health insurance programs:
Health Plan. 18.01 The Company will pay full 100% Ontario Health Tax costs as provided by the Province of Ontario Medicare Programme during the life of the Collective Agreement. The Company shall maintain Medicare coverage and benefits in force as of October 1, 1992.
