Health Plan. The City shall provide a group comprehensive health care plan to regular employees. The City shall pay one hundred percent (100%) of the monthly cost of single coverage. The City will pay ninety-four percent (94%) of the monthly cost of family coverage and ninety-four percent (94%) of the monthly cost of 2/4 party coverage.
Health Plan. (a) Producers, the IATSE, and the respective West Coast Locals are parties to the "Agreement and Declaration of Trust establishing the Health Plan for the Employees of the Motion Picture Industry" (hereinafter referred to as the "Health Plan") made as of October 20, 1952. Such Health Plan was established in accordance with the provisions of the "Producer-I.A.T.S.E. and M.P.M.O. Supplemental Agreement of October 25, 1951."
Health Plan. The Department will contribute on behalf of eligible employees in this Unit, to whom an annual salary rate is applicable, and who are members of the Water and Power Employees’ Retirement, Disability and Death Benefit Plan, and on behalf of their eligible dependents, if any, a sum not to exceed $823.76 a month, through June 30, 2005, toward the cost of any one of the following health insurance programs:
Health Plan. Manager may participate in Health and Dependent Care flexible spending account programs in effect and generally available to other employees of the District. The District shall contribute flexible dollars and non-elective contributions towards Manager’s health insurance as described in the District’s Section 125 Cafeteria Plan document. Total contributions (flexible dollars plus non-elective contributions) shall equal the lowest cost HMO health care plan less the following amounts on a monthly basis: Manager only: $0 Manager plus one dependent: $61 per month Manager plus 2 or more dependents: $100 per month If the Manager provides the District with sufficient evidence of outside health insurance coverage that meets the District approval, the District shall add $200 per month to the Manager’s base pay in lieu of providing health insurance coverage.
Health Plan. So long as the Employer offers the Standard Benefits Plans in accordance with this Agreement, the Employer shall share with each eligible employee who elects to participate in a Health Plan the cost of the premiums for the plan in which the employee elects to participate. The rate that the Employer and the Employee shall pay is the Standard Sodexo rates. The Employer shall deduct the employee’s share of the premium from each paycheck on a pre- tax basis.
Health Plan. The University will provide the same health plan options to all eligible members of the bargaining unit with the same plan design as are offered to the University’s other faculty, non-represented and represented staff, subject to the University’s right to amend the plan, so long as such amendments are applied equally to the members of the bargaining unit and to the University’s other faculty and non-represented staff.
