Child or Children refers to all young people who have not yet reached their 18th birthday.
If it has been indicated that Children will be a part of this marriage, then establish whether the Child or Children belongs to the “Couple” or if these Children were had separately.
In the case of a Qualifying Event that is Medicare entitlement of the covered Employee or former Employee, date of entitlement, and name(s) and address(es) of Spouse and dependent Child or Children covered under the Plan.
