Health and Hospitalization Insurance. Single Coverage: The District shall contribute a sum not to exceed $8180 per year toward the premium for individual coverage for each full-time employee employed by the District who qualifies for and is enrolled in single cov- erage in the School District’s group health and hospitalization insurance plan. Any additional cost of the premium shall be borne by the employee and paid by payroll deduction.

Family Coverage: The School District shall contribute a sum not to exceed $586.00 per month toward the premium for family coverage for each full-time employee employed by the School District who qualifies for and is enrolled in family coverage in the School District’s group health and hospitalization insurance plan. The school employer will pay up to the following listed amounts annually toward the cost of hospital, surgical, and medical care type insurance for each full-time teacher employed under regular or temporary contract and enrolled in the school corporation's group plan effective January 01: Any increase and or reduction in the premium above said Board's annual contribution shall be shared equally between the Board and teacher. Up to the amount specified will be paid to such insurance company or companies as is determined and selected solely by the school employer with said teacher paying not less than one dollar ($1.00) annually. No change in the policy or benefits shall be made during the term of this contract. Should both a husband and wife, who are employed in a certified position, be entitled to participate in this insurance plan, such persons may elect to: A. enroll in two (2) separate single plans, B. enroll in one (1) family membership, or
Health and Hospitalization Insurance. Subd. 1. Effective July 1, 2019, the School District shall make an annual contribution toward the cost of the premium for medical/hospitalization insurance on behalf of each teacher employed by the School District who qualifies for and is enrolled in one of the group medical/hospitalization plans offered. The amount of this contribution shall depend on the plan the teacher selects from among the group plans available through the School District. See Appendix K, page 48, for plan options and corresponding District contributions for each plan.

Family Coverage: The District shall contribute a sum not to exceed $14,212 per year toward the premium for family coverage for each full-time employee employed by the District who qualifies for and is enrolled in family cov- erage in the District’s group health and hospitalization insurance plan. Any additional cost of the premium shall be borne by the employee and paid by payroll deduction. In no event shall the District’s contribution for a husband and wife working combination exceed the family coverage contribution provided for 1 Maintenance/IT employee of the District.
Health and Hospitalization Insurance. Eligibility. All employees working an average of 30 or more hours per week in accordance with the Affordable Care Act are eligible for benefits in this Article.

Health and Hospitalization Insurance. The School District shall make available a high deductible health plan to all employees who elect to participate in said plan. With respect to qualifying bargaining unit members, the School District shall contribute an amount not to exceed $9,423.08 towards the annual premium cost of single, subscriber plus one or family group health insurance coverage for school years 2022-2023 and $$9,923.08 2023-2024. After the premium cost has been paid, remaining contribution dollars will be placed in the employees XXXX account.
