Plan Design. Details of the plans are contained in brochures provided by the Employer and include: • Medical Service Plan of British ColumbiaExtended Health Benefit PlanGroup Life InsuranceAccidental Death and Dismemberment Plan (AD&D) • Short-Term Disability Plan (STD) • Long-Term Disability Plan (LTD) • Dental Plan (Option 3)  75% Part A (Basic Preventative)  75% Part B (Major Restorative)  50% Part C (Orthodontics) • Employee and Family Assistance Plan (EFAP)
Plan Design means the covered prescription benefits, participant contributions and limitations adopted by the Sponsor as outlined in Schedule A of this Agreement.
Plan Design means Program drug coverage, days’ supply limitation, Copayment/Coinsurance, Formulary (including Formulary drug selection) and other Program specifications applicable to the Program designated by SPONSOR as set forth in this Agreement or otherwise documented between the parties.

  • The Prescription Drug Benefit Program may be modified by the State Health Benefits Plan Design Committee, pursuant to its authority under P.L. 2011, c.

  • The Dental Care Plan may be modified by the State Health Benefits Plan Design Committee, pursuant to its authority under P.L. 2011, c.

  • Each Stand Alone Dental Plan (SADP) offered by Contractor under the terms of this Agreement shall provide dental benefits in accordance with the Benefit Plan Design requirements as required under this Agreement, and as applicable, any other laws, rules and regulations, including California Health and Safety Code § 1367.005, California Insurance Code § 10112.27, California Government Code § 100503(e), 45 C.F.R. § 155.1065, 45 C.F.R § 156.150, and as applicable, 45 C.F.R. § 156.200(b).

  • The State Health Benefits Plan Design Committee shall provide to employees the option to select one of at least three levels of coverage each for family, individual, individual and spouse, and individual and dependent, or equivalent categories, for each plan offered by the program to employees of the Judiciary, differentiated by out of pocket costs to employees including co-payments and deductibles.

  • Until such time as different contribution levels are mandated through legislation or are established by the SHBP or the State Health Benefits Plan Design Committee, those employees who had accrued less than 20 years of creditable service as of June 28, 2011, and who accrue 25 years of pension credit or retire on disability retirement on or after July 1, 2011, will contribute toward the cost of post retirement medical benefits in accordance with the grid established by P.L. 2011, c.

  • By virtue of the size, complexity and fluctuating nature of the workload associated with the implementation of the Ranch Plan and other projects, the demands of County staff in performing the Planning, Subdivision, Engineering/Architecture, and CEQA Plan Review and professional development processing of Plan Design Review will be extraordinary.

  • The Plan Design for each of these plans is attached and made part of this agreement as Exhibit A.

  • The Health Care Plan Design shall be in accordance with Appendix “D”.

  • It is agreed that the STATE shall continue the Dental Care Plan during the period of this Agreement to the extent it is established and/or modified by the State Health Benefits Plan Design Committee, in accordance with P.L. 2011, c.


More Definitions of Plan Design

Plan Design means the Covered Services, exclusions, co-payment, formulary (including formulary drug selection cost indications, and formulary compliance programs), and other specifications applicable to each Group.
Plan Design means drug coverage, days’ supply limitation, Cost Share, Formulary (including Formulary drug selection and relative cost indication) and other Prescription Drug Program specifications applicable to the Prescription Drug Program designated for Client as set forth in this Agreement or otherwise documented between the Parties.
Plan Design means the Pharmacy Benefit Plan Design Form as agreed upon by both parties which includes the setup of the Group Health Plan.
Plan Design means the group dental insurance benefits, as set forth in Attachment C: Plan Design. “Plan Year” is based on the calendar year from January 1 to December 31.

Related to Plan Design

  • Plan Proponents means the Debtors and the Committee.

  • EHC plan means an Education, Health and Care plan made under sections 37(2) of the Children and Families Act 2014.

  • Separation Plan means the Company’s Separation Plan Amended and Restated Effective August 13, 2006, as may be amended from time to time or any successor plan, program, arrangement or agreement thereto.

  • Medical Executive Committee or “MEC” means the Executive Committee of the Medical Staff.

  • Plan A means the 1997 Incentive Stock Option Plan;
