PURPOSE AND DESCRIPTION OF SERVICES. The Calabasas Civic Center consists of two buildings built in 2008: a one-story 25,059 square foot Library and a two-story 27,733 square foot City Hall, which are LEED (Leadership in Energy and Environmental Design) certified with a rating of Gold. Specialty requirements, which are incorporated into the requirements of the RFP, are involved in maintaining the LEED certification of Gold. The City of Calabasas has implemented a “green” cleaning and building maintenance program that both increases health/safety for occupants and maintenance staff and minimizes harmful effects to the environment. The program will satisfy these goals by reducing exposure of building occupants and maintenance staff to harmful airborne elements commonly associated with building operations and maintenance activities (such as particulate matter, hazardous chemicals, and biological hazards), and by implementing a maintenance program that reduces impact on the environment through careful selection of cleansing agents, sustainable cleaning practices and use of recycled paper products. The City of Calabasas is seeking proposals from companies to provide “green” janitorial services for the Calabasas City Hall and Library, which includes a public meeting room and public restrooms.

PURPOSE AND DESCRIPTION OF SERVICES. The California Department of Corrections and Rehabilitation, California Correctional Health Care Services, herein referred to as “CDCR/CCHCS”, has identified a need for Oxygen Manifold Removal, Purchase, and Installation Services at High Desert State Prison (HDSP). The oxygen manifold is located in Xxxx 000 of the Central Health Building. The Contractor agrees to provide removal of existing Ohmeda Oxygen Manifold, purchase and installation of the new Oxygen Manifold Unit, (Automatic Dome Loaded Digital) and Header Bar Assembly (including Copper Pigtails). The contractor is responsible for installation of electrical power, testing and certification of the new Oxygen Manifold Unit. CDCR/CCHCS assumes no responsibility for any work commenced by the Contractor and will not reimburse the Contractor for any work performed prior to approval from the CDCR/CCHCS Contract Manager or designee. The CDCR/CCHCS Contract Manager or designee shall be solely responsible for determining acceptability and satisfactory completion of any services provided by the provider and shall authorize all invoices. Testing and certification shall be based on industry standards requiring a written document from the vendor stating the completion of ‘successful testing and subsequent certification of the system for operation per industry standards’. Services shall be performed only by Contractor staff and/or subcontractors. Apprentices trainees are not acceptable.
PURPOSE AND DESCRIPTION OF SERVICES. The purpose of this SOW is to outline the scope for the implementation and ongoing support of Ellevation. To the extent you require implementation services over the course of the agreement term, Ellevation’s Partner Support Organization utilizes a six-phase implementation process with clearly defined steps, deliverables, and timeframes to deploy Ellevation. User Agencies will request services through the execution of a “Subsidiary Agreement” from the awarded contractor for the requested services in the appropriate Tier corresponding to the Number of Eligible Persons. User Agencies are defined as a state agency or local governmental entity (any city and/or county, district, or other local governmental body or corporation, including the California State Universities and University of California systems, K-12 schools and community colleges empowered to expend public funds). Use of this MSA is optional by User Agencies. The State of California (hereinafter referred to as “State”) does not guarantee the number of User Agency orders over the term of this MSA. The term of the resulting MSA will be for three (3) years with two (2) optional one-year extensions at the option of the State, as set forth in the sample agreement and its exhibits. All Subsidiary Agreements and orders issued against this MSA prior to the MSA End Date must still be completed in its entirety. All services must be performed within the continental United States of America.

PURPOSE AND DESCRIPTION OF SERVICES. The California Department of Corrections and Rehabilitation, California Correctional Health Care Services (CDCR/CCHCS) has identified a need for a Mental Health Diversion: Consultation, Technical Assistance and Policy Recommendation in critical roles that support the adoption of best practices throughout California for the Council of Criminal Justice and Behavioral Health (CCJBH). CCJBH serves as a resource to assist and advise the administration and Legislature on best practices to reduce the incarceration of youth and adults with mental illness and substance use disorders with a focus on prevention, diversion and reentry strategies. Specifically, CCJBH’s mission is “…to strive to end the criminalization of individuals with behavioral health challenges by supporting proven strategies that promote early intervention, access to effective treatments, a planned re-entry and the preservation of public safety.” A key strategy to meet CCJBH’s mission is California’s recent investments in mental health diversion programs, which have begun to serve as an off-ramp from the criminal justice system to community-based care, thereby reducing incarceration and recidivism among individuals living with behavioral health challenges. To support California’s mental health diversion efforts, CCJBH was provided with resources for three (3) years to provide consultation to Department of State Hospitals (DSH) and California counties to develop, implement and operate pre-trial mental health diversion programs. As part of this effort, this contract for subject matter expert specialty services, will strengthen CCJBH’s objective to support the statewide adoption of diversion best practices, including developing policy recommendations to sustain and expand efforts in the wake of the COVID-19 public health emergency.

It is the intent of Department of General Services (DGS), Procurement Division (PD) to award multiple MSAs to responsive and responsible bidders. A responsive bidder is a bidder whose solicitation response is compliant with solicitation requirements and is fully capable of performing the specified MSA services. Requesting Agencies defined as a state agency or local governmental entity (any city and/or county, district, or other local governmental body or corporation, including the California State Universities and University of California systems, K-12 schools and community colleges empowered to expend public funds) will request services through the execution of a “Subscription Agreement” from a list of contractors ranked by Region and Category or through issuance of a Request for Offer. Use of this MSA is optional by Requesting Agencies. The term of the resulting MSA contracts will be for three (3) years with two (2) one- year optional extensions at the option of the State, as set forth in the sample contract and its exhibits. All Subscription Agreements and orders issued against this MSA must be completed within twelve (12) months following the expiration of this MSA contract term. This MSA excludes work in a California Superior Court. All services must be performed within the continental United States of America. Refer to the Proposed Form of Agreement, Exhibit A, which is attached to this IFB as Attachment 13 for a more complete description of services.
