Change Order Form. The Change Order Form is the instrument by which adjustments in the Contract Price and Contract Time are effected pursuant to changes made in accordance with Case (a), Case (b) or Case (c) of Article 29 of the General Conditions or in accordance with Subparagraph (i) of Article 29 of the General Conditions. The Change Order Form shall be accompanied by a breakdown in the form prescribed in a specimen, which the Owner will supply to the Contractor. The Engineer shall certify to the amount of the adjustment. The Change Order Form shall be signed by the Contractor and the Owner. The breakdown is only for the purpose of enabling the Engineer and the Owner to make a judgment on the dollar amount of the adjustment in the Contract Price. No condition, term, qualification, limitation, exception, exemption, modification, or proviso shall appear in the breakdown. The breakdown shall be in the exact form and language of the above-mentioned specimen. In the event any condition, term, qualification, limitation, exception, exemption, modification, or proviso shall appear in a breakdown, it shall be invalid.

Change Order Form. No instructions, either written or verbal, shall be construed as an order for changes unless it be in the form of a Change Order, bearing the signed approval of the COUNTY and the COUNTY’S representative and the signed acceptance of the CONTRACTOR’S signature to the order will not be mandatory. Change Orders shall describe or enumerate the work to be performed and state the price, if any, to be added to or deducted from the Agreement sum. Except as provided in the above paragraph, no change shall be made, unless in pursuance of a Change Order, and no claim for an addition to the Contract sum shall be valid unless so ordered. If the CONTRACTOR believes that any instructions, by drawing or otherwise, involves extra cost under his/her Agreement, he/she shall give the COUNTY written notice thereof and await instructions before proceeding to execute such work.
Change Order Form. The Construction Manager shall submit five (5) originals of the Change Order Form (OEF 425), signed by the Architect to the Owner’s Project Representative. The Change Order Form shall be executed by the Architect, Construction Manager and Owner. The Architect of Record for the Project shall certify in writing, and attach to the Change Order, that the cost of the requested change is fair, reasonable, and in proper proportion to the cost of the original work of the contract and shall recommend action thereon.
