Coverage and Benefit. This protection plan covers the Chromebook loaned to the student against all accidental damage, theft, vandalism or loss for one (1) school year. This program does NOT cover damage or loss of the power cord, which we ask students to keep at home. This program does NOT cover intentional misuse, abuse or neglect by any household members. If the student did not exercise proper care and/or take appropriate precautions, as described in the student handbook, and this behavior resulted in damage to or loss of the Equipment, the cost of the repair or replacement will be the responsibility of the student/parent. Upon paying for the Chromebook Protection Plan, if the Chromebook is accidentally damaged or vandalized, it will be repaired as shown in the table below. The power cable and case are the replacement responsibility of the user. SLSD will accept only Chromebook approved adapters and cables on the return of the Chromebook. Effective and Expiration This coverage is effective from the date this required form and premium payments are received by the school through the date at which the Chromebook is required to be returned in good order to the school. Premium The total Technology Protection Fund cost is $30.00 per school year. It is agreed and understood that: ● The $30 Technology Protection Fund payment is required for repair/replacement coverage. ○ This covers accidental damage/repairs ○ If the $30 fee is not paid, students will be responsible for at cost repairs (Market Price) ● A separate application will be needed for each Chromebook covered. ● If a student maliciously damages a Chromebook beyond repair due to negligence, loses the Chromebook, or has it stolen, the premium will be raised to $300 for a replacement Chromebook. ● If the chromebook or its replacement is damaged, lost or stolen a second time, the student will be charged full price for the device. ● Any Chromebook intentionally damaged will result in full replacement cost of the device. ● In the event an Chromebook is stolen, a police report must be filed and a copy provided to the school administration before a replacement is issued. It will be the responsibility of the school administration to determine if damages were due to negligence or accidental. A student with a damaged Chromebook will be given a replacement. If the Chromebook or its replacement is damaged, lost or stolen a second time, the student will not be permitted to take the Chromebook outside of the building for one calendar year and will be charged full price for the device. Coverage and Benefit. This agreement covers the computer loaned to the student against failure and/or damage over $100.00. The agreement covers the equipment 24 hours per day both inside and outside of school. Total repair/replacement costs will be determined at the time repairs are made. In the event of accidental damage or malfunction, the user will be responsible for 100% of the repair costs up to a maximum of $100.00. Payment for repairs will be due at the time repairs are made. Theft claims for the first occurrence will be subject to $200.00 maximum whereas a second theft second claim will be subject to full replacement cost or $405. Any careless, malicious or intentional damage to the equipment is not covered by this policy and will require the full replacement cost to be borne by the parent on behalf of the student. This policy does not cover lost equipment. Accessories valued less than $50.00 are not covered by this agreement. In no case shall the value be greater than $405.
Coverage and Benefit. This agreement covers the iPad loaned to the student for the 2020-2021 school year against incidental damage or loss over $50. Accessories (charger and case) and damages valued at less than $50 are NOT covered and are the sole responsibility of the student. Coverage is 24 hours per day. The Technology Protection Plan will only cover two incidents for the school year. NEGLECT IS NOT COVERED. If paid, the Technology Protection Plan will release the parent/guardian from having to pay for loss or damage for the following reasons: • Damages resulting from normal use and accidents. • Loss or damages resulting from theft. An official copy of a police report is required. • Loss or damages resulting from a fire. An official fire report from the investigating authority is required. • Loss or damages resulting from a natural disaster. • Damages resulting from a power surge. • Damages resulting from vandalism by another individual. If found guilty, the vandal will be liable to pay for damages. Exclusions (parent/guardian will be liable for entire replacement cost of the device): • Intentional damages. • Lying on any damage report. • Not carrying the iPad in a district-provided protective case. • Negligence (including loss from carelessness). • Loss or damages resulting from fraudulent, intentional, or criminal acts. Effective and Expiration This coverage is effective from the date premium payment is received by the school through the date at which the iPad is required to be returned in good working order. Premium The total premium cost for the 2020-2021 school year is $25. The Technology Protection Plan is not refundable. Payments can be made through XxXxxxxxxxXxxx.xxx until August 31 or by check made out to Xxxxxxxx City Schools. If paying by check, please write your student’s ID number and TPP in the memo line on your check and turn it in to your school office. It is agreed and understood that: • The HCSD Technology Protection Plan will offer coverage to all students. • Participation is totally voluntary. If a student does not participate, they will be responsible for any costs associated with repairs, loss, or damage. • A separate premium will be needed for each iPad covered. The Building Administration will determine if damages were due to negligence or accident. The Technology Protection Plan will only cover two incidences per school year. Coverage will be in effect upon payment of the premium. The Technology Protection Plan will follow Board Policy-JN.

Coverage and Benefit. The technology protection fund agreement covers all listed repairs and replacement charges for the first repair/replacement of each item listed. The coverage is 24 hours per day. All subsequent repairs and replacement charges are not covered under this fund and will be charged to the student. • Participation in the Technology Protection Fund is optional. • Coverage under the Technology Protection Fund begins on the date the parent completes the Technology Protection Fund Form and expires at the end of the school year, or the first day of school the following school year if it needs to be kept over the summer. If the student enrolls after the beginning of the school year, the student has up to 30 days from the enrollment date to enroll in the protection fund.
Coverage and Benefit. The district acknowledges that due to the normal usage of district technology that maintenance and/or repair of district technology may be necessary due to no fault of the student. These routine maintenance issues and malfunctions that are not due to “visible physical damage” of the MacBook Air will be completed at no cost to the student. The following policy applies to accidental damage to the MacBook Air. *The AC adapter is not included in this weighted repair policy. In the event of a lost or damaged adapter, the Xxxxxx Technology staff will replace the adapter and the full replacement cost will be the responsibility of the borrower.
