WEEKLY INCOME INSURANCE. If you are unable to earn your living because of an accident or illness, your Weekly Income Insurance would provide you with a weekly income. (See the Schedule of Benefits at the front of this booklet for the amount.) • To receive Weekly Income benefits you need not be confined at home, but your disability must be severe enough to prevent you from performing your regular work, and you must be under the continuous care and personal attendance of a physician. • Weekly Income benefits begin with the first day of disability due to injury or the fourth day of disability due to illness. If you have not seen a physician on or before the date benefits would otherwise start, they will not start until after your first visit to the physician. • Weekly Income benefits will be paid for a total of not more than 4 weeks for each period of disability. • Benefits will not be paid for: - disability due to injury sustained while working for pay or profit. - disability due to or associated with treatment rendered for aesthetic purposes. - disability during a period you are serving a prison sentence. - disability during the scheduled duration of a leave of absence including maternity leave. Maternity leave is considered to begin on the earlier of the date agreed upon by you and your employer or the date of birth. This limitation does not apply to any portion of a period of maternity leave during which you are disabled due to pregnancy - disability during the scheduled duration of any lay-off unless you become disabled

WEEKLY INCOME INSURANCE. 5.01 An insured employee who is totally disabled and unable to work due to sickness or an accident not covered by Workers' Compensation, will be paid a weekly benefit commencing on the first day of absence due to an accident and the fourth day of absence due to a sickness, but in no event prior to the date of the first treatment by the attending physician.

The amount of weekly income benefit shall be equal to of an employees weekly insurable earnings (as defined under the Unemployment Insurance Act of Canada), provided such weekly insurable earnings do not exceed the maximum weekly insurable earnings under the Unemployment Insurance Act of Canada in effect on the date of calculation. In no event shall the amount of Weekly Income exceed the amount permitted under said Unemployment Insurance Act. Weekly Income benefits will be paid for a total of not more than weeks for each period of disability. It is not required that the insured employee be confined to home, but the employee must be unable to perform his regular duties and under the continuous care and attendance of a licensed physician during the period for which benefits are being claimed or the duties associated within a work accommodation which the union and the company agree are suitable. Successive periods of disability shall be considered as one period of disability, unless the employee returns to work and completes at least two (2) complete consecutive weeks of active, full-time employment before the commencement of the later disability or unless the later disability is due to causes wholly different from those of the prior disability and commences after the employee has returned to work. Weekly Income benefits are not payable for the following: Disability due to injury sustained while working for pay or profit or illness for which the employee is covered under Workers Compensation or similar law; or Disability due to or associated with treatment rendered for aesthetic purposes; or Disability during a period the employee is serving a prison sentence; or Disability resulting from self-inflicted injury, war, or engaging in a riot or insurrection; or Pregnancy where a female employee is not disabled; or During the period which commences with the 10th week prior to the expected week of confinement for pregnancy and which ends with the 6th week after the week of such confinement; or Any period during which a female employee is on a pregnancy leave of absence in accordance with Article of this Agreement.

The terms and provisions of such coverage shall be made available to the Union effective September 1, 1992. Short-Term Disability Plan: Waiting Period - 7 calendar days Percent of Pay - 66-2/3 percent of base pay Maximum - $3,000.00 Per Month Maximum Time - 6 months Long-Term Disability Plan: Waiting Period - 6 months Percent of Pay - 66-2/3 percent of base pay Maximum - $3,000.00 Per Month Maximum Time - 2 years The terms, conditions, exclusions, and limitations specified in the County's policy with its insurance carrier shall govern all conditions of eligibility for and payment of benefits. Changes in benefit amounts based on changes in an employee's base weekly earnings occur on each March 1 based on the employee's base weekly earnings as of the preceding January 1st. Any employee whose personal disability extends beyond the period compensated and the employee is not able to assume the responsibilities of his position, his seniority will terminate. Vacancies that occur due to an employee's being on weekly income insurance may be filled by the hiring of a person outside of the bargaining unit for a period of up to ninety (90) calendar days which period may be extended by mutual consent of the Union and the County.

  • Basic Life Insurance 37.1 The Employer shall pay one hundred percent (100%) of the monthly premium of the basic life insurance plan.

  • Dependent Life Insurance In the event of the death of your spouse or dependent child from any cause whatsoever, while you and your dependents are insured under the plan, the insurance company will pay you $10,000 in respect of your spouse and $5,000 in respect of each insured dependent child. This applies to those employees with family health coverage only.

  • Medical/Dental Expense Account The Employer agrees to allow insurance eligible employees to participate in a medical and dental expense reimbursement program to cover co- payments, deductibles and other medical and dental expenses or expenses for services not covered by health or dental insurance on a pre-tax basis as permitted by law or regulation, up to the maximum amount of salary reduction contributions allowed per calendar year under Section 125 of the Internal Revenue Code or other applicable federal law.
