For purposes of benefits 24 provided under the SEBB, the eligibility year is the school year from September 1 25 through August 31. 26 27 The District will implement the School Employees Health Care Coalition 28 agreement when collecting the employee premiums which will be paid to the 29 Health Care Authority (HCA) through payroll deduction for benefits coverage. 31 Benefits offered to employees through SEBB at the present time include: 32 • Basic Life and accidental death and dismemberment insurance (AD&D) 33 • Basic Long-term Disability 34 • Vision 35 • Dental including orthodontia 36 • Medical Plan 37 38 Employees are eligible to participate in the Medical Flexible Spending 39 Arrangement (FSA) and Dependent Care Assistance Program (DCAP) offered by 40 SEBB. Employees will also have the option of enrolling in a Health Savings 41 Account (HSA) when they select a qualifying High Deductible Health Plan 42 (HDHP) for their medical insurance. In addition, employees will be able to utilize 43 payroll deduction for any supplemental insurance that they choose to enroll in 44 through SEBB (e.g. increased Life, AD&D, Long-term disability, etc.). 46 B. Eligibility: 47 48 All employees shall be eligible for full insurance coverage under the SEBB 49 program if they work, or are anticipated to work, 630 hours or more in an 50 eligibility year. 52 Should an employee who previously was not expected to be eligible for benefits 53 under SEBB work 630 hours in one year, benefits coverage will begin on the first 54 day of the month following the date they actually work 630 hours for the year.

Statement of Work (SOW) – A detailed description of the work activities the Contractor is required to perform under the terms and conditions of this Contract, including the deliverables and timeline, and is Schedule A hereto. Subcontractor – A person, partnership, or entity not in the employ of or owned by the Contractor, who is performing all or part of those services under a separate contract with or on behalf of the Bidder. The terms “Subcontractor” mean Subcontractors in any tier. Tax-advantaged accounts – Pre-tax or tax-exempt accounts used to pay for qualified medical and dependent care expenses. For purposes of this Contract, HBAs are tax-advantaged accounts. United States Code (USC) – All references in this Contract to USC chapters or sections will include any successor, amended, or replacement statute. The USC may be accessed at xxxx://xxxxxx.xxxxx.xxx/ Washington Administrative Code (WAC) – Any references to specific titles, chapters, or sections of the WAC includes any substitute, successor, or replacement title, chapter, or section. SPECIAL TERMS AND CONDITIONS PERFORMANCE EXPECTATIONS Expected performance under this Contract includes, but is not limited to, the following: Knowledge of applicable state and federal laws and regulations pertaining to subject of contract; Use of professional judgment; Collaboration with HCA staff in Contractor’s conduct of the services; Conformance with HCA directions regarding the delivery of the services; Timely, accurate and informed communications; Regular completion and updating of project plans, reports, documentation and communications; Regular, punctual attendance at all meetings; and Provision of high-quality services. Prior to payment of invoices, HCA will review and evaluate the performance of Contractor in accordance with Contract and these performance expectations and may withhold payment if expectations are not met or Contractor’s performance is unsatisfactory. TERM The initial term of the Contract will commence on date of last signature and continue through December 31, 2026, unless terminated sooner as provided herein. HCA reserves the right, in its sole discretion, to extend this Contract for six (6) periods of up to one (1) year each. School Employees Benefits Board (SEBB. Eligibility: Employees shall be deemed eligible for medical and non-medical benefits if they are expected to work a minimum of six hundred thirty hours (630) per year or the State mandated minimum for SEBB benefits, whichever is less. Employees who are not expected to work enough hours in order to be initially eligible, but who work at least the number of hours to be eligible during the contract year, shall become eligible to enroll for benefits in the current contract year upon reaching the eligibility threshold of hours and deemed eligible for the remaining contract year. This provision shall include substitute employees.
