Wellness Bonus. Employees who have perfect attendance for a period of two (2) consecutive calendar months will be given the opportunity of using one (1) day of any accumulated sick hours for a Wellness Day. This is a day off which is requested three (3) weeks in advance with the supervisor and the supervisor shall respond in one (1) week from the date the request was submitted. Such day must be taken within the following three (3) calendar months and must not be taken on a holiday. Your scheduled Wellness Day is not considered as an absent day.

Wellness Bonus. Each employee (even those who do not receive their health benefits through the City and have accepted a cashout under 14.5) will be eligible for up to $400.00 per year as a wellness bonus. Wellness bonuses are available if an employee provides proof of participation in the following wellness activities in the amounts stated:
Wellness Bonus. ‌ The wellness bonus is provided to active members of the CBU for the purpose of maintaining their physical well-being and remaining physically fit as defined by the Contract. Employees who resign in good standing during the bonus period and meet all other qualifications at the time of resignation will receive a prorated payment for the time worked during the bonus period. Employees who are terminated or resign in lieu of termination during the bonus period are not eligible for the wellness bonus. Part-time members of the CBU will receive a prorated portion of the wellness bonus beginning with their first eligible quarter. The wellness bonus is paid at $1000.00 for a period year and is paid on a quarterly basis at the end of each quarter. The bonus amounts represent gross dollars and are subject to taxes. Payments will be made during the next full pay period after the quarter ends. Qualification -must be verified prior to the quarterly payments. The criteria for the wellness bonus will be posted and remain, in Armories, Break rooms, and on Union Bulletin Boards. The Company understands that the awarding of the bonus pursuant to this section may require the Company to increase the regular rate of pay for the calculation of overtime and/or double time earned and paid during the bonus performance period. As of the first bonus period calculated after December 15, 2019, the Company will determine if the awarding of a bonus causes the need to adjust overtime and/or double time premium earnings, and the increased amount will be paid to the affected Employees in the pay period following the bonus payment.
Wellness Bonus. Unit members who take zero (0) days of Personal Necessity Leave (Article 10.8) or sick leave (Article 10.9) in a contract year shall be entitled to a bonus of two-hundred fifty dollars ($250) payable on a paycheck by the end of the fiscal year. Unit members who take one (1) day of Personal Necessity Leave (Article 10.8) or sick leave (Article 10.9) in a contract year shall be entitled to a bonus of one-hundred twenty-five dollars ($125) payable on a paycheck by the end of the fiscal year.
Wellness Bonus. In the event that a Member works a twelve (12) month period, concurrent with the calendar year, and takes zero (0) hours of sick leave during said calendar year, the Member shall receive a "wellness" bonus of $300.00. In the event that a Member works said calendar year and takes one shift or less (i.e. 8 hours or less or 10 hours or less, depending on scheduling) of sick leave during said calendar year, the Member shall receive a "wellness" bonus of $150.00. Said bonuses shall be paid during the month of February of the year following. A Member who is hired during the calendar year or terminated during the calendar year shall not be eligible for "wellness" bonus pay.

  • SHIFT BONUS 7:01 A day shift shall be a shift that commences after 4:30 a.m. and at or before 10:00 a.m. on the same day.

  • Wellness Incentive Employees participating in the State’s medical plan and who meet the wellness criteria established by the State, in consultation with the Union, shall receive a reduction in medical insurance co-share payments up to a maximum of $500 per year. The earned reductions in medical insurance co-share payments shall be awarded to active employees in FY 2009 or the fiscal year following the employee’s participation in the wellness activities. The Wellness Incentive program will integrate preventative and wellness behaviors into the medical plan. Examples of possible activities include completion of the Health Assessment, obtaining a primary care physician, wellness coaching programs, preventive screenings, non-smoker or completion of smoking cessation program, and/or participation in a program that measures key points in assessing an individual’s overall health.

  • Longevity Bonus Effective 2005, twenty (20) years of continuous service, an employee will receive a longevity bonus of seven hundred dollars ($700.00) per year, payable in one lump sum by the second pay period following the employee's anniversary date.

  • Sick Leave Bonus For every six (6) months of perfect sick leave attendance after July 1, 1987, the employee will receive eight (8) hours of bonus time. This bonus time will be prorated for part-time employees. Such bonus time can be used for any leave purpose covered by this Agreement. Such bonus time shall be counted as vacation leave credits for purposes of determining eligibility for carry- over and cash payments.

  • CHRISTMAS BONUS 16.01 All part-time employees on the payroll of the Company as of December 1st in any year who have completed six (6) months' continuous service with the Company shall be entitled to a Christmas Bonus of fifteen ($l5.00) dollars payable on or before December 15th.

  • Vacation Bonus Employees shall receive one day's base pay (or adjusted earnings) for each year of service beyond twenty-five (25) years, to a maximum of ten (10) days’ pay.

  • Bonus The Executive shall be eligible for Bonuses determined by the Board.

  • Bilingual Bonus A seven percent (7%) bilingual bonus will be paid to those employees who in the performance of their duties are required by the Employer to use the second official language on a regular and consistent basis.

  • Fringe Benefits During the Employment Period, the Executive shall be entitled to fringe benefits, including, without limitation, tax and financial planning services, payment of club dues, and, if applicable, use of an automobile and payment of related expenses, in accordance with the most favorable plans, practices, programs and policies of the Company and its affiliated companies in effect for the Executive at any time during the 120-day period immediately preceding the Effective Date or, if more favorable to the Executive, as in effect generally at any time thereafter with respect to other peer executives of the Company and its affiliated companies.

  • Performance Bonus If Employee's employment is terminated by Employee with cause, or by Bank without cause, Employee shall be paid, in addition to the amounts payable under Sections 3.5 and 3.6 of the Agreement: (i) all non-forfeitable deferred compensation, if any; and (ii) unpaid performance bonus payments, if any, payable under Section 4.2 of the Agreement, which shall be declared earned and payable based upon performance up to, and shall be pro-rated as of, the date of termination. Employee shall not be entitled to such unpaid performance bonus payments if Employee's employment is terminated by Bank with cause, or by Employee without cause.
