Wellness Incentive. The HSA plan set forth in this Article shall include a wellness incentive program, designed to provide early diagnosis and appropriate information to patients so that they and their health care professionals can determine appropriate, timely courses of treatment as needed. The wellness program will include preventive physical examinations. If the administrator and the administrator’s spouse (if applicable) complete one preventive physical examination during the term of the contract, the Board will make a one-time contribution into the administrator’s HSA, in the amount of five percent (5%) of the applicable deductible under the HSA plan. For the purposes of this paragraph, the measurement period for completing the physical examination will be the calendar year. The Board will make its additional five percent (5%) HSA contributions on or about the July 1st following completion of the calendar year during which the physical exams are completed.
Wellness Incentive. The health insurance plan set forth in this Article shall include a wellness incentive program, designed to provide early diagnosis and appropriate information to patients so that they and their health care professionals can determine appropriate, timely courses of treatment as needed. The wellness program will include age-based preventive physical examinations and age-based preventive screenings. If a teacher and the teacher’s spouse (as applicable) fulfill all applicable requirements of the wellness program for a plan year, the Board will contribute an additional five percent (5%) of the applicable deductible under the HSA plan into the teacher’s HSA for the following year.
Wellness Incentive. If no more than three (3) days of sick leave are used the employee will receive a wellness incentive payment of $130.00. If four (4) days of sick leave are used then the incentive payment will $80.00. If five (5) days are used then the incentive payment will be $60.00. When six (6) or more sick days are used, there will be no wellness incentive payment. In order to receive payment, a claim voucher must be completed and submitted to the District Office by the last day of school.

More Definitions of Wellness Incentive

Wellness Incentive. Employees, employees’ spouses, retirees, and retirees’ spouses covered by a City health insurance plan who complete a Health Risk Assessment (HRA) with the City Health Department and who participate in one additional wellness program during the same year will receive an incentive payment, payable to the employee’s City-provided flexible spending account or directly to the employee via payroll, by the City. The election as to payment to the flexible spending plan or directly to the employee must be made at the time of completion of the HRA. An employee or retired employee shall be eligible for a $200 wellness incentive payment. Employees’ spouses or retired employees’ spouses shall be eligible for a $100 wellness incentive payment. Employees and retirees will be eligible for no more than two payments per family per year. The City may, by policy, provide additional Wellness Incentives at the City’s discretion. Employees shall be permitted to complete the HRA while on paid status consistent with the work needs of the department for which the employee works. Employees shall not be on paid status to complete the HRA during times for which the employee is not scheduled to work. The HRA includes a health risk appraisal, blood lipid profile, blood pressure reading, and a consultation with a medical professional.
Wellness Incentive. Any member is eligible to participate in the incentive program, receiving a $100 per eight (8) hours buy-back for up to thirty two (32) hours of unused sick leave per school year. Requests to receive the buy-back days must be submitted to the human resources department by May 31. Employees must have a 403(b) plan or establish one to participate. Payment for unused sick leave will be placed annually in the teachers’ 403(b) plan. This payment is not matched by the District. Eligibility for the incentives will be based on usage of sick leave in the previous year and based on the following criteria: Sick leave days used in previous year: Number of days for qualified payment: Zero sick hours used Thirty two (32) hours payment at $100 per eight (8) hours Up to 16 sick hours used Sixteen (16) hours payment at $100 per eight (8) hours Payment to the member’s 403(b) plan shall be made on or before June 30th. Donation of sick / emergency leave hours to Section 3 - Voluntary Sick Leave Pool will not count against qualifying for this wellness incentive.
Wellness Incentive. Effective January 1, 2022, a member who successfully qualifies for the Service-approved Fitness Pin shall be awarded one (1) day of paid leave credited in OSL. For clarity, one day shall be equal to the member’s daily hours worked based on their current schedule. A member can qualify for this incentive only once every twelve (12) months. Such day shall be taken off within twelve (12) months of when it was earned.

  • Long-Term Incentive Award means an Award described in Section 6(g) hereof that is based upon a period in excess of one year.

  • Bonus means the bonus described in the Section 2.3.

  • Performance Compensation Award means any Award designated by the Committee as a Performance Compensation Award pursuant to Section 11 of the Plan.

  • Management Incentive Plan means the Company’s bonus program, as implemented by the Company’s board of directors from time to time and pursuant to which the Executive may receive incentive-based compensation at fiscal year end.

  • Recoverable Incentive Compensation means Incentive Compensation received by a Covered Officer during the Lookback Period that exceeds the amount of Incentive Compensation that would have been received had such amount been determined based on the Accounting Restatement, computed without regard to any taxes paid (i.e., on a gross basis without regard to tax withholdings and other deductions). For any compensation plans or programs that take into account Incentive Compensation, the amount of Recoverable Incentive Compensation for purposes of this Policy shall include, without limitation, the amount contributed to any notional account based on Recoverable Incentive Compensation and any earnings to date on that notional amount. For any Incentive Compensation that is based on stock price or TSR, where the Recoverable Incentive Compensation is not subject to mathematical recalculation directly from the information in an Accounting Restatement, the Administrator will determine the amount of Recoverable Incentive Compensation based on a reasonable estimate of the effect of the Accounting Restatement on the stock price or TSR upon which the Incentive Compensation was received. The Company shall maintain documentation of the determination of that reasonable estimate and provide such documentation to the Exchange in accordance with the Listing Standards.

  • Average Incentive Compensation means the average of the annual incentive compensation under Subparagraph 3(a) received by Executive for the three (3) immediately preceding fiscal years or such fewer number of complete fiscal years as Executive may have been employed by the Company. In no event shall “Average Incentive Compensation” include any sign-on bonus, retention bonus or any other special bonus. Notwithstanding the foregoing, if the Executive breaches any of the provisions contained in Paragraphs 4 and 5 of this Agreement, all payments of the Severance Amount shall immediately cease. Notwithstanding the foregoing, in the event Executive terminates his employment for Good Reason as provided in Subparagraph 6(e), he shall be entitled to the Severance Amount only if he provides the Notice of Termination provided for in Subparagraph 6(f) within thirty (30) days after the occurrence of the event or events which constitute such Good Reason as specified in clauses (A), (B), (C), (D), (E) and (F) of Subparagraph 6(e);

  • Incentive Pay means an annual amount equal to the aggregate annual bonus, in addition to Base Pay, made or to be made in regard to services rendered in any calendar year or performance period pursuant to any bonus plan of the Company.
