Wellness Incentive definition

Wellness Incentive. If no more than three (3) days of sick leave are used the employee will receive a wellness incentive payment of $130.00. If four (4) days of sick leave are used then the incentive payment will $80.00. If five (5) days are used then the incentive payment will be $60.00. When six (6) or more sick days are used, there will be no wellness incentive payment. In order to receive payment, a claim voucher must be completed and submitted to the District Office by the last day of school.
Wellness Incentive. The HSA plan set forth in this Article shall include a wellness incentive program, designed to provide early diagnosis and appropriate information to patients so that they and their health care professionals can determine appropriate, timely courses of treatment as needed. The wellness program will include preventive physical examinations. If the administrator and the administrator’s spouse (if applicable) complete one preventive physical examination during the term of the contract, the Board will make a one-time contribution into the administrator’s HSA, in the amount of five percent (5%) of the applicable deductible under the HSA plan. For the purposes of this paragraph, the measurement period for completing the physical examination will be the calendar year. The Board will make its additional five percent (5%) HSA contributions on or about the July 1st following completion of the calendar year during which the physical exams are completed.
Wellness Incentive. Employees, employees’ spouses, retirees, and retirees’ spouses covered by a City health insurance plan who complete a Health Risk Assessment (HRA) with the City Health Department and who participate in one additional wellness program during the same year will receive an incentive payment, payable to the employee’s City-provided flexible spending account or directly to the employee via payroll, by the City. The election as to payment to the flexible spending plan or directly to the employee must be made at the time of completion of the HRA. An employee or retired employee shall be eligible for a $200 wellness incentive payment. Employees’ spouses or retired employees’ spouses shall be eligible for a $100 wellness incentive payment. Employees and retirees will be eligible for no more than two payments per family per year. The City may, by policy, provide additional Wellness Incentives at the City’s discretion. Employees shall be permitted to complete the HRA while on paid status consistent with the work needs of the department for which the employee works. Employees shall not be on paid status to complete the HRA during times for which the employee is not scheduled to work. The HRA includes a health risk appraisal, blood lipid profile, blood pressure reading, and a consultation with a medical professional.
