Health Benefits and Life Insurance. A bargaining unit employee is eligible for enrollment in any insurance plan whether life or health offered to the employees of the City of Northampton. During the term of this contract, the District agrees to pay at least fifty percent (50%) of health and life insurance premiums for plans offered to the employees of the City of Northampton, except that the District agrees to pay eighty percent (80%) of the premiums for hospital and medical coverage in the City of Northampton’s group plans provided through health maintenance organizations. The District will provide a $5,000 life and accidental death and dismemberment policy to bargaining unit members.

The Company will provide through an insurance carrier the following life insurance and health coverages, the details of which are outlined in an employee benefits handbook provided by the insurance carrier: Company to maintain employee premiums in current state for 2010 and 2011. Adjustment to cost sharing from 75/25 to 80/20 for year 3. Prescription Drugs - Employee and dependents - $10.00 deductible per prescription Semi-Private - Employee and dependents - 100% premiums Company paid Life Insurance - $30,000 employee life insurance - $15,000 each dependent life insurance - 100% premiums Company paid Accidental Death and Dismemberment - $30,000 employee - Per specific loss schedule - 100% premiums Company paid Extended Health (Podiatrist, Chiropractic, Physiotherapy, Ambulatory etc.) - Employee and dependents Dental - Employee and dependents - current-year ODA Schedule Vision Care - adults - three hundreds dollars ($300.00) every two (2) calendar years - children - one hundred and fifty dollars ($150.00) every two (2) calendar years - eye exams – company will pay on a two-year basis to a maximum of $100.00. In order to qualify employee will have to be enrolled in the benefit package.

A health insurance, prescription drug, dental benefit and vision benefit program as described in the City of Moline Health Benefit Plan booklet (hereinafter referred to as “health insurance program”) shall be offered to all permanent employees. The actual plan documents shall be the basis of any final interpretation or eligibility and benefits. Effective January 1, 2019, employees shall pay 20% of the total monthly premium. The amount of the City’s contribution is based on the selection of the “medical” enrollment category. If an employee elects “employee only” medical coverage, then the “employee only” allowance is provided to the employee. Any contribution not utilized by the employee shall revert to the City.
