Deluxe Travel Plan. As well as the above, the Employer shall provide a dental plan known as “Liberty Health #9” at the current ODA fee schedule, by a carrier acceptable to the parties to this Collective Agreement provided the retiree pays one hundred (100) percent of the cost of the premium to carry this benefit. Following the ten (10) year period, the employee may request continued coverage, at their own expense. It is the responsibility of the employee to keep the Employer informed of their current address and to submit such a request. The Employer's premium assistance may stop or be reduced if any government premium assistance is available, and the employee shall make the necessary application for such assistance. Normal retirement for the purpose of this article shall mean on or after age fifty-five (55), to age sixty-five (65), and as defined by Ontario Municipal Employees Retirement System. The benefits to be provided to retired employees shall be those benefits outlined in this section (Article 19.02) of the Collective Agreement as amended from time to time and is subject to Article 19.04.

Deluxe Travel Plan. In the event that the level of benefit changes for current employees, the same change will apply to retired employees. This coverage will remain in effect from the date of normal retirement through to the lesser of: Age 65; or for a period of ten (l0) years. Effective January 1, 2012, as well as the above, the retiree is eligible to participate in the dental plan provided to current employees provided the retiree pays thirty (30) percent of the cost of the premium to carry this benefit. At the conclusion of the benefit period, the employee may request continued coverage, at their own expense. It is the responsibility of the employee to keep the Employer informed of their current address and to submit such a request. The Benefits provided shall include the cost of their annual fee billed by the Provincial Government for retired employees to have access to the Ontario Drug Benefit Program. The benefits provided shall also provide for the costs of prescriptions.
Deluxe Travel Plan. 12. Group Life Insurance for Eligible members = 3 times the member’s annual salary to the nearest $1,000. A member can purchase additional life insurance in accordance with the master plan, maximum to an amount equal to two (2) times the member’s annual salary, at the member’s own expense, subject to minimum participation required by the Carrier. Dependent life coverage is available at the member’s own expense in the amount of a spousal benefit of $20,000 and a child benefit of $5,000

  • Leave Donation Program Employees may donate paid leave to a fellow employee who is otherwise eligible to accrue and use sick leave and is employed by the same Agency. The intent of the leave donation program is to allow employees to voluntarily provide assistance to their co-workers who are in critical need of leave due to the serious illness or injury of the employee or a member of the employee’s immediate family. The definition of immediate family as provided in rule 123:1-47-01 of the Administrative Code shall apply for the leave donation program.

  • Meal Plan The Student who resides in a university residence hall is required to purchase a full residential dining plan (commuter plans are not acceptable).   • Medical Plan ‌ Eligible employees and dependants shall be covered by the British Columbia Medical Services Plan or carrier approved by the British Columbia Medical Services Commission. An eligible employee who wishes to have coverage for other than dependants may do so provided the Medical Plan is agreeable and the extra premium is paid by the employee through payroll deduction. Membership shall be a condition of employment for eligible employees who shall be enrolled for coverage following the completion of three (3) months’ employment or upon the initial date of employment for those employees with portable service as outlined in Article 14.12.

  • Orientation Program The Company will allow a designated representative of the Local or Bargaining Unit up to one (1) hour per calendar month for the purpose of conducting the Communications, Energy and Paperworkers Union New Members’ Orientation Program. Such meetings will be conducted during the probationary period of employees, and will be held on Company premises. Employees participating in Orientation Program meetings during their normally scheduled working hours will not suffer loss of pay at their regular rate. Orientation Program meetings will be scheduled by Management and a Management representative may attend as an observer.

  • Sick Leave Donation Program A Labor Management Committee will be established for the purpose of proposing rules and procedures for a new, program. The LMC will be to develop consistent, transparent and equitable proposals for processes across all departments within the City. The LMC shall also explore proposals to lower the minimum leave bank required to donate sick leave and permit donation of sick leave upon separation from the City. The LMC must consult with the Office of Civil Rights to ensure compliance with the City’s Race and Social Justice Initiative. Once the LMC has developed its list of proposals, the City and Coalition of City Unions agrees to reopen each contract on this subject.

  • Vision Care Plan The County agrees to provide a Vision Care Plan for all employees and dependents. The Plan will be the Vision Service Plan - Plan A with benefits at 12/12/24 month intervals and with twenty dollar ($20.00) deductible for examinations and twenty dollar ($20.00) deductible for materials. The County will fully pay the monthly premium for the employee and dependents and pick up inflationary costs during the term of the Agreement.
