Employee Roster. After receipt of award and prior to starting work aboard the vessel, the contractor must submit a list of employees who will work aboard ship to the Commanding Officer of the ship via the NSA Security Office. The list should be on company letterhead, include each employee's name and security clearance when required, and bear the signature of a company official.
Employee Roster. The Municipality agrees to furnish the Association once each quarter with a roster of all employees working for the Department when same is required by the Association. The Association agrees that it will furnish all forms required by the Municipality to be used in complying with the provisions of this section.

The County shall supply to the Union on a biweekly basis a listing of extra-help employees and their length of service, grouped by department. For extra-help employees hired sixty (60) days following the adoption of this Agreement, hours listed will include only hours worked in an extra-help, rather than regular, capacity, except for persons who fluctuate between regular and extra-help status during the course of their County employment. The County shall notify the Union of extra-help employees who are on a leave of absence status in excess of 28 days. Such lists shall be supplied without cost to the Union, provided that Union-requested changes to the report will be charged to the Union.
Employee Roster. Semi-annually, in the months of November and May, the Hospital will provide to the Association, via electronic spreadsheet attachment to email, a complete list of employees covered by this Agreement. The list will include the name, address, home telephone, budgeted hours, shift, department, employee identification number, hourly rate of pay, most recent date of hire into the bargaining unit and original date of hire for each employee. In addition, the Hospital will provide to the Association, via electronic spreadsheet attachment to email, a monthly roster of new hires and terminations that includes the name, address, home telephone, budgeted hours, shift, department, employee identification number, hourly rate of pay, most recent date of hire into the bargaining unit and original date of hire.
Employee Roster. Each month the Employer shall provide the Union with a list of names, addresses, phone numbers, social security numbers, job classification, department or work area, shift, date of hire, rate of pay, and FTE status of those employees covered by this Agreement. Each month the Employer will also provide the Union with a list of names and addresses of new hires, terminations, and transfers into/out of the bargaining unit, along with date of hire/termination/transfer. Both of these lists shall be transmitted to the Union electronically in a mutually agreeable format.
Employee Roster. A roster shall be furnished by the County at the beginning of each year which lists the name, job title, date of hire and job location of each employee in the bargaining unit. An updated list will be furnished every six (6) months to FOP 112 at its request.

The Employer shall submit to the Union a monthly roster of all employees covered by this Agreement including their job classification, worksite, rate of pay, hours worked, FTE status, starting date, and date of birth. The Employer will also submit to the Union the home address, personal email, and personal phone number of all newly hired employees into the bargaining unit and any bargaining unit employees who have been terminated or are on a leave of absence.
