Employee Insurance Benefits. 2 The District shall provide negotiated insurance benefits as specified within the 3 master contract(s) between the District and the respective insurance providers the 4 District selects. A brief summary of those benefits is included in brochures provided 5 by the insurance providers. Those summaries provided by the insurance providers 6 are based upon the terms and conditions of the master insurance policy contract(s) 7 maintained in the Human Resources Division of the Education Center.
Employee Insurance Benefits. 24.1 The City agrees to provide medical, hospitalization, dental, and optical insurance programs for employees covered by this Agreement and their eligible dependents. In the event that the City should desire to change to different insurance plans or insurance carriers for any of the provided insurance programs, the City and the Union shall jointly meet and review the coverage proposed by the new carrier(s).
Employee Insurance Benefits. A. Upon acceptance of written application by the insurance carriers, employees shall become eligible for insurance benefit programs beginning the first day of the month following employment. The employee must be actively at work on the day that the coverage becomes effective. Board contributions for coverage shall cease the end of the month following termination or retirement. Changes in family status shall be promptly reported by the employee to the Employee Benefits Office within 30 days of such change. The employee shall be responsible for any overpayment of premiums made by the Board in his/her behalf for failure to comply with this paragraph.

For the term of this Agreement, the Company will maintain and make available to employees plans consisting of medical benefits, dental benefits, vision, prescription drug benefits, life insurance and accidental death and dismemberment benefits, and long-term disability benefits, as described to the Union in negotiations, or similar benefits. All plans will be administered solely in accordance with the provisions of each plan. The selection of the plan administrator, the administration of the plans and all the terms and conditions relating thereto, and the resolution of any disputes involving the terms, conditions, interpretation, administration, or benefits payable shall be determined by and at the sole discretion of the Company. The Company shall have the right to amend the plans in any way, including the selection of carriers. Any amendment to the level of benefits or Company HSA contribution or the cost of benefits will be the same as those changes applicable to non-bargaining employees. The benefit levels and costs of all insurance benefits and Company HSA contribution offered by the Company will be the same as those offered to non-bargaining employees. The employee’s contribution toward the cost of such plan will be the same as that of a similarly situated non-bargaining unit employee electing the same coverage. When any changes in such benefits are to be made, the Company will give the Union at least thirty (30) days’ notice of such changes and upon request will meet to discuss the reason for the change.

Effective as of July 1, 2012, the Board's monthly contributions towards the cost of medical insurance shall be limited to the following amounts: $458.33 Single; $916.66 Two person; $1,250 Family. The annual amount of the Board's contribution towards medical insurance shall not exceed $5,500 Single; $11,000 Two Person; $15,000 Family.
