Credit Card Policy. I understand that TCFAP requires a credit card on file. My insurance requires payment at time of service for all deductibles, co-pays, and coinsurance. As a courtesy, TCFAP will keep my credit card on file and process payment when the EOB is received from my insurance. Otherwise all charges must be settled at the time of service. NO EXCEPTIONS to this policy. I understand it is my responsibility to checkout at the time of visit if I refuse to leave my credit card on file. If I do not settle at check-out and do not have a credit card on file, my account will be considered past due and a $25 fee will be incurred. I understand it is my responsibility to update my credit card on file when it expires or is replaced. I understand that having a credit card on file ensures payment and allows me additional time to settle my account and that credit cards are stored electronically and are encrypted. State and federally funded insured are not required to leave a credit card on file as there is no balance billing. I understand that any patient balance under $250 will be automatically collected using the credit card on file. I understand any patient balance over $250 will be collected after 14 days if the balance has not been disputed. I understand that if I am a non-TCFAP patient/visitor or using one of TCFAP additional services beyond primary care (Behavior Health, Lactation Services, Travel Medicine, Nutrition and/or any future services that might be offered) I am required to place my credit card on file. Credit Card Policy. For your convenience, Xxxxxx accepts credit card payments and offers Vacation Rental Insurance through third party processors. OWNER’s tenants have the option to utilize an online ACH or credit card process service to pay their rent using Visa, Mastercard or Discover. If they elect to use this option a ($2.95) transaction fee and a convenience fee of 3% of the funds due will be added to their charge. This Credit Card Authorization and the Lease are binding contracts. By signing and / or supplying Rand and London with their credit card information, Lessee / Cardholder agrees to the terms and conditions herewith and to the terms and conditions of rental of Rand & London's Lease Agreement. Failure to separately sign either document does not ameliorate or cancel the other or invalidate the mutual obligations to each other, or negate either in any way. Funds to cover your Card purchases will be deducted from your checking account. If the balance in your account is not sufficient to pay the transaction amount, the Credit Union may pay the amount and treat the transaction as a request to transfer funds from other deposit accounts, approved overdraft protection accounts or loan accounts that you have established with the Credit Union. If you initiate a transaction that overdraws your Account, you agree to make immediate payment of any overdrafts together with any service charges to the Credit Union. In the event of repeated overdrafts, the Credit Union may terminate all services under this Agreement. You may use your Card and PIN (Personal Identification Number) in ATMs of the Credit Union, Plus, VISA, CO-OP, and ACCEL networks, and such other machines or facilities as the Credit Union may designate. The following limitations on the frequency and amount of Visa Debit Card transactions may apply: • You may make 15 Debit Card purchases per day. • You may purchase up to a maximum of $3,500.00 per day. • You may make 15 cash withdrawals in any one (1) day from an ATM machine. • You may withdraw up to a maximum of $500.00 in any one (1) day from an ATM machine, if there are sufficient funds in your account. • You may make 15 POS transactions in any one (1) day. • You may purchase up to a maximum of $3,500.00 from POS terminals per day, if there are sufficient funds in your account. • You may transfer up to the available balance in your accounts at the time of the transfer. • If you incur a charge in a currency other than U.S. dollars, the charge will be converted into the US dollar amount. Please see the Foreign Transactions section below. Also, transactions in certain countries may be blocked and will require a manual or verbal authorization for approval. Please check with us prior to departure to determine if your destination country is atfected. However, the right to deny such transactions is within our sole discretion and atfected countries may change at any time.

  • Special Rule for Credit Card Purchases If you have a problem with the quality of property or services that you purchased with a credit card, and you have tried in good faith to correct the problem with the merchant, you may have the right not to pay the remaining amount due on the property or services. There are two limitations on this right:

  • Credit Card Authorization TO THE EXTENT PERMITTED BY APPLICABLE LAW, YOU IRREVOCABLY AND UNCONDITIONALLY AUTHORIZE XXXXXXX TO CHARGE ALL AMOUNTS DUE UNDER THIS AGREEMENT TO ANY CREDIT CARD YOU PROVIDE TO US, AND YOU AGREE TO INDEMNIFY, DEFEND AND HOLD HARMLESS XXXXXXX WITH RESPECT TO THE SAME.

  • Schedule of Underlying Insurance Policies If Umbrella or Excess policy is evidenced to comply with minimum limits, a copy of the Underlying Schedule from the Umbrella or Excess insurance policy may be required.

  • Agreement with Respect to Credit Card Business The Assuming Bank agrees to honor and perform, from and after Bank Closing, all duties and obligations with respect to the Failed Bank's credit card business, and/or processing related to credit cards, if any, and assumes all outstanding extensions of credit with respect thereto.
