Foundation Report. Yeh will prepare a Foundation Report for Earth Retaining Systems (FR) only for the design of the proposed retaining wall along the northbound US 101 off-ramp to Xxxxx Street. The scope of the design geotechnical services should be reviewed and updated as needed based on the alternative selected for the interchange improvements and the preliminary design of the improvements. This work will include planning, field exploration, laboratory testing and performing geotechnical analyses for preparation of the Foundation Report. The scope of the field exploration and testing programs assumes that the work will be performed under a single mobilization and effort.
Foundation Report. Subsurface exploration to adequately define earth materials and foundation conditions is expected to include three sampled, logged test borings: one at each abutment and at the channel support to approximately 80-ft depth to satisfy the suggested foundation types in the RFP. Two boreholes will also be completed in the roadway to approximately 5 to 10-ft depths to evaluate subgrade materials for pavement section analysis. Bulk soil samples will be taken from the roadway boreholes for later analysis and testing. Exploration Details: We propose to make the test borings from existing roadway, which will require at least partial lane closures; we assume that warning signs and traffic cones will provide adequate traffic control with flaggers as necessary. Xxxxx will provide the traffic control. We will notify USA for location of buried utilities before starting field exploration. Xxxxx will apply for environmental and encroachment permits for this work. We assume that the encroachment permit will be a no fee permit. For the channel boring, it is assumed that access to the channel can be made with a crawler drill rig. Access may require minor backhoe work which is assumed provided by the County. Additionally, a Fish and Game permit will be required to drill in the channel. Typical work windows for in-channel work are between June 1st and October 00xx.Xxxx and Game permits also typically take approximately 3 months to process. The permit process should be started immediately after notice to proceed to allow time for processing and acceptance by the department. We propose to recover soil samples from the borings at 5±ft intervals using Standard Penetration equipment. The encountered soils will be field classified and borings logged (including groundwater conditions) by an engineer/geologist. Laboratory testing to supplement field evaluation of soils parameters is expected to include moisture-density and unconfined compressive strength determinations and corrosivity testing (pH/minimum Resistivity/sulfate/chloride content) on selected samples. “R”-value testing will be performed on bulk soils samples in evaluation of subgrade materials. The scope of our proposed services specifically excludes sampling and testing for the presence or distribution of hazardous materials. If hazardous materials are identified during field exploration, work will be stopped at that location and the client notified; a modified scope of services may be required. Foundation Report. The additional scope of services outlined below are based on the parties understanding of the project components as of the date of this proposal and consist of performing observation and logging during site specific field exploration, laboratory testing and preparation of Log of Test Borings (LOTB) sheets as the basis for preparing the FR-ERS. CONTRACTOR will coordinate a site visit with Granite Construction (under CMGC Preconstruction Services contract). CONTRACTOR will review field conditions, traffic control, closures, and work hours. CONTRACTOR will xxxx the locations of the explorations in the field. CONTRACTOR will coordinate and schedule the field exploration program with Granite Construction. CONTRACTOR will provide geotechnical field logging and sampling services and field oversight during exploration with drilling and CPT subcontractors. CONTRACTOR understands that Granite Construction will retain drilling and CPT subcontractors for the project work and that exploration will be performed as close as practical to the proposed wall alignment as possible. CONTRACTOR will perform laboratory tests on selected samples collected from the drilling. Soil tests for classification, shear strength, corrosion, and consolidation will be performed on selected samples recovered from the borings. The types and numbers of tests will be selected based on the results of the field exploration program. Lastly, CONTRACTOR will prepare Log of Test boring sheets for the project plans in accordance with Caltrans guidelines and logging procedures.

  • Evaluation Report 20.5.4.1 The summary evaluation report shall be prepared by the Faculty Evaluation Committee and administrative evaluator(s) and shall include each evaluator’s individual rating. The summary evaluation report shall take into account the results of each of the evaluation components (Section 20.5.3) in order to arrive at an overall rating. When the committee and the administrative evaluator(s) cannot reach an agreement as to the overall rating, the report must include written explanation.

  • Project Reports; Completion Report 1. The Recipient shall monitor and evaluate the progress of the Project and prepare Project Reports in accordance with the provisions of Section 2.06 of the Standard Conditions and on the basis of indicators agreed with the World Bank. Each Project Report shall cover the period of one (1) calendar semester, and shall be furnished to the World Bank not later than one (1) month after the end of the period covered by such report.

  • Evaluation Reports Where a formal evaluation of an employee's performance is carried out, the employee shall be provided with a copy to read and review. Provision shall be made on the evaluation form for an employee to sign it. The form shall provide for the employee's signature in two (2) places, one indicating that the employee has read and accepts the evaluation, and the other indicating that the employee disagrees with the evaluation. The employee shall sign in one of the places provided within seven (7) calendar days. No employee may initiate a grievance regarding the contents of an evaluation report unless the signature indicates disagreement with the evaluation. The employee shall receive a copy of the evaluation report at the time of signing. An evaluation report shall not be changed after an employee has signed it, without the knowledge of the employee, and any such changes shall be subject to the grievance procedure.

  • Construction Reports Each Constructing Entity shall issue reports to each other Construction Party on a monthly basis, and at such other times as reasonably requested, regarding the status of the construction and installation of the Interconnection Facilities. Each Construction Party shall promptly identify, and shall notify each other Construction Party of, any event that the Construction Party reasonably expects may delay completion, or may significantly increase the cost, of the Interconnection Facilities. Should a Construction Party report such an event, Transmission Provider shall, within fifteen days of such notification, convene a technical meeting of the Construction Parties to evaluate schedule alternatives.

  • Project Completion Report At the completion of construction and once a Project is placed in service, the Subrecipient must submit a Project Completion Report that includes the total number of units built and leased, affordable units built and leased, DR-MHP units built and leased, an accomplishment narrative, and the tenants names, demographics and income for each DR-MHP unit.

  • Utilization Reports Both the Contractor and Subcontractor shall complete Quarterly Utilization Reports (or similar type documents containing the same information) and submit them to the Contracting Officer and to the Bureau within ten (10) business days at the end of each quarter. This information will be used to determine the actual dollar amount paid to Subcontractor and will also serve as a record of fulfillment of Contractor’s Small Diverse Business and Small Business Commitments. If there was no activity during the quarter, then the form must be completed by stating “No activity in this quarter.” A late fee of $100.00 per day may be assessed against the Contractor if its Utilization Report is not submitted in accordance with the schedule above.
