An Evaluation Report clause outlines the requirements and procedures for preparing and submitting a formal assessment of a project's progress, performance, or compliance. Typically, this clause specifies who is responsible for creating the report, the criteria or metrics to be evaluated, and the timeline for submission. For example, it may require a contractor to provide a detailed summary of completed work and any issues encountered at regular intervals. Evaluation Report. The state must provide a narrative summary of the evaluation design, status (including evaluation activities and findings to date), and plans for evaluation activities during the extension period. The narrative is to include, but not be limited to, describing the hypotheses being tested and any results available.
Evaluation Report. Using an approved HHSC template, the Grantee will collaborate with their external, subcontracted evaluator to provide an annual evaluation plan of the yearly programmatic activities.
Evaluation Report. The Classroom/Workplace Observation and Evaluation Form (C2) shall be completed by the School ▇▇▇▇ or approved Administrator subsequent to the classroom/workplace observation and shall include the following:
1. Identification of faculty member’s strengths.
2. Identification of areas needing improvement 3. General overall comments
Evaluation Report. In preparing the Evaluation Report for placement in the unit member’s personnel file, the evaluator shall rely primarily upon data collected through observations, which may include inspecting the employee’s work while being performed and/or after a job is completed and may include conferences with employee to discuss work performance issues prior to the formal evaluation process. Any deficiencies which may have been brought to the attention of the unit member and corrected may not be negatively referenced but may be constructively addressed.
Evaluation Report. 20.8.3.1 The summary evaluation report shall be prepared by the Faculty Evaluation Committee and first-level manager or designee and include each evaluator’s individual rating. The summary evaluation report shall take into account the results of each of the evaluation components (Section 20.8.2) in order to arrive at an overall rating. When the committee and the first-level manager or designee cannot reach an agreement as to the overall rating, the report must include a written explanation.
20.8.3.2 For satisfactory evaluations only, evaluators may note less-than-serious deficiencies that do not warrant an improvement plan. The first-level manager shall monitor improvement of noted less-than-serious deficiencies until the evaluatee’s next evaluation.
Evaluation Report. A. The administrator/principal or immediate supervisor will review the Evaluation Report Form for the employee's personnel file at the conference. The final evaluation conference shall be held no later than five (5) school days following the final observation of the employee.
B. The employee's signature on the Evaluation Report Form shall not necessarily indicate concurrence with the evaluation. The evaluee has the exclusive right to attach an addendum statement.
C. Distribution of the final Evaluation Report Form shall be:
1. original - school district personnel file;
2. first copy - principal; and
3. second copy - employee
Evaluation Report. (1) An evaluation report shall be completed by the DPC on the standardized form attached as an appendix to this Agreement, and shall be an evaluation of the Member's performance during the appraisal period set out in Clause 14.17. When evaluating Members, the DPC shall determine evidentiary requirements subject to Clause 14.18. The evaluation shall be based on the information submitted in the Member's activity report and, where relevant to evaluating the Member’s performance based on the criteria, the Member’s prior evaluation report(s) for up to three (3) years. The DPC may request further information from the Member to aid in the interpretation and evaluation of the report when the activity report is not reasoned, transparent or clear.
(a) Each DPC and Chair shall formulate criteria of qualitative and/or quantitative measurement in the three areas of evaluation, and shall provide them in writing to the ▇▇▇▇ for review. In formulating these qualitative and/or quantitative measurements, the DPC and Chair shall have regard to the Faculty-based criteria for continuing appointment and promotion established by the ▇▇▇▇ for the Faculty pursuant to Clauses 24.10 and 25.18. The ▇▇▇▇ shall review the criteria drafted by the Chair and DPC to ensure that they are consistent with the Faculty-based criteria. Where the ▇▇▇▇ considers these criteria to require further review, clarification and/or changes, the ▇▇▇▇, the Chair and the DPC shall consult to determine appropriate criteria.
(b) Once approved by the ▇▇▇▇, these criteria of qualitative and/or quantitative measurement may be reviewed by the Chair and DPC every five (5) years.
(c) The agreed upon criteria shall be provided by the Chair to the Members and shall be applicable in each subsequent Academic Year until revised in accordance with the Agreement.
(a) The DPC shall evaluate the Member’s performance according to the terms of Clause 14.18 (1)-(3) and shall, in each of the three categories, indicate the level at which the Member has performed as per the following terms:
(b) The DPC shall provide its rationale for the evaluation in each category.
(4) The DPC shall also provide an overall evaluation of the Member’s performance using the terms specified above. The DPC may recommend that a Member receive a Merit Award pursuant to Clause 28.08.

Evaluation Report. A written report of this evaluation shall be transmitted concurrently to the Faculty Member, Department Head, and ▇▇▇▇ of the College or ▇▇▇▇▇▇▇ and Vice President, and entered in the Faculty Member's evaluation file. Subdivision
Evaluation Report. A Member who submits an Activity Report shall be evaluated at least once per Academic Year. The evaluation shall include a review of all Senate-approved Course/Instructor evaluations for the course(s) taught by the Member, and include other such information as submitted with the Activity Report which reflects evidence of the Member’s teaching during the review period. The Chair/Director or equivalent and one other Regular Academic Staff member shall conduct the evaluation, and shall make their evaluation based upon the Member’s Activity Report, and such other information that is supportable by documentation that was made available for written response by the Member using the form appended to this Agreement. The evaluation shall be completed by April 1. The completed evaluation(s) shall be provided to the Member and the Member shall sign the form acknowledging that he/she has read the evaluation(s).
Evaluation Report. Newly employed APAs/Clerks must have a written evaluation of their work performance by their immediate supervisor within one year following employment. All APAs/Clerk will be evaluated at least once every three (3) years. The written evaluation will be given to the employee and they may choose to write an attachment to clarify any difference of perception in the evaluation. If areas of serious concern are noted in the evaluation, a plan of improvement will be written and shared with the employee. The employee will be given a reasonable time to correct these areas. No APA shall evaluate or type the evaluation of another APA. An APA will acknowledge receipt of an evaluation by signing and dating a disclaimer clause at the bottom of the evaluation.
