Proof of Sickness Sample Clauses

Proof of Sickness. Sick leave with pay is only payable because of sickness or injury and employees who are absent from duty because of sickness may be required by the Employer to prove sickness. Failure to meet this requirement can be cause for disciplinary action. Repeated failure to meet this requirement can lead to dismissal. A doctor’s certificate may be requested for each leave of more than three (3) consecutive work days.

Proof of Sickness. Provided that no employee employed under the provisions of this Agreement shall be entitled to the benefits of this clause unless he/she produces proof of his/her sickness satisfactory to this employer. If requested such proof of sickness may be in the form of a medical certificate.

Proof of Sickness. An employee may be required to produce a certificate from a qualified medical practitioner for any sickness in excess of three (3) working days, certifying that such employee is unable to carry out his duties due to sickness, or non-compensable accident. Where an employee incurs a cost in obtaining such medical certificate the Board shall reimburse such cost upon presentation of receipt.

Proof of Sickness. 9.03 A member who has been absent for three (3) consecutive days on sick leave may be required, upon the written request of the Chief Constable, to produce a certificate from a duly qualified medical practitioner for any illness, such certificate to state that the member is unable to carry out his duties due to illness.
