Xxxx Leave Without Pay. Upon written request of an employee, the Human Resources Director may grant leave without pay due to personal illness/injury for up to thirty (30) calendar days and may grant extensions up to a total maximum period of ninety (90) calendar days. Extensions of sick leave without pay up to a maximum of an additional ninety (90) calendar days must be approved by the City Manager. The initial and subsequent requests for sick leave without pay must be accompanied by a physician's certificate stating the nature and approximate duration of medical disability.
Xxxx Leave Without Pay. Upon application by the employee, sick leave without pay may be granted or renewed by the Employer for up to six (6) months after accrued sick leave, vacation and holiday time have been exhausted. When the employee goes into sick leave without pay status the employee will not accrue benefits. The Employer may require that the employee submit a certificate from a physician periodically during the period of such disability.
Xxxx Leave Without Pay. 1. A sick leave without pay absence may only be granted when an employee has exhausted all accrued sick leave with pay and accrued annual leave.
Xxxx Leave Without Pay. Regular employees may be granted a sick leave of absence without pay for the period of disability due to a personal illness or injury. If verification is required, the employee will have ten (10) calendar days to provide the required documentation. The duration of such unpaid leave of absence is subject to the limitations of Xxxxxx Transit’s operating needs and applicable laws, approved at the sole discretion of Xxxxxx Transit. Once a leave of absence without pay is approved, periodically, Xxxxxx Transit’s Executive Director of Administration, or designee, may require the employee to submit a certificate from the employee’s health care professional or from a designated health care professional verifying the need for continued leave. In the event of a failure or refusal to supply such certificate, or if the certificate does not clearly show sufficient disability to preclude the employee from the performance of essential duties, Xxxxxx Transit may cancel such leave of absence and require the employee to report for duty on a specified date. Employees absent due to illness or injury or to care for a family member, and who have insufficient accrued paid sick leave to cover the entire absence, will move into a sick leave without pay status for any full or partial day absence. Exceptions, to include the use of other accrued paid leave such as vacation or personal holiday in lieu of sick leave without pay, must be authorized by the employee’s Executive Director or designee. Once the employee is in a sick leave without pay status, not previously approved by Xxxxxx Transit, each absence is considered an unexcused absence.

After earned sick leave has been exhausted, the Agency shall grant sick leave without pay for any job-incurred injury or illness for a period which shall terminate upon demand by the teacher for reinstatement accompanied by a certificate issued by the duly licensed attending physician that the teacher is physically and/or mentally able to perform the duties of the position. After earned sick leave has been exhausted, the Agency shall grant sick leave without pay for any job-incurred injury or illness for a period which shall terminate upon demand by the teacher for reinstatement accompanied by a certificate issued by the duly licensed attending physician that the teacher is physically and/or mentally able to perform the duties of the position. After earned sick leave has been exhausted, the Agency may grant sick leave without pay for any non-job-incurred injury or illness to any teacher upon request for a period up to one (1) year. Extensions of sick leave without pay for a non-job-incurred injury or illness beyond one (1) year must be approved by the Agency and the administrator. The Agency or the administrator may require that the teacher submit a certificate from the attending physician or practitioner in verification of disability resulting from a job-incurred or non-job-incurred injury or illness. Any cost associated with the supplying of a certificate concerning a job-incurred injury or illness that is not covered by Workers' Compensation benefits shall be borne by the Agency. Any cost associated with the supplying of a certificate concerning a non-job-incurred injury or illness shall be borne by the teacher. In the event of a failure or refusal to supply such a certificate, or if the certificate does not clearly show sufficient disability to preclude that teacher from the performance of duties, such sick leave may be canceled and the teacher's service terminated.
Xxxx Leave Without Pay. Upon application of a regular employee, a leave of absence without pay may be granted by a hiring authority for the entire period of disability because of sickness or injury. Such leave need not be limited to one (1) year, but the hiring authority or the Executive Director of Employee Services or designee from time to time, may require that the employee submit a certificate from the attending medical professional or from a designated medical professional. In event of a failure or refusal to supply such certificate or if the certificate does not clearly show sufficient disability to preclude employees from the performance of their duties, the hiring authority, with the approval of the Executive Director of Employee Services, may cancel such sick leave and require the employee to report for duty on a specified date.
Xxxx Leave Without Pay. Sick leave without pay may be taken in accordance with the provisions of the Family & Medical Leave Act.
