Weekend, holiday and evening service will be performed only in the event of a Mechanical or Electrical Breakdown of a Covered Product, for which repair is prescribed under this Agreement, which is essential to Your health and safety (“Emergency Repair”). An event will qualify for Emergency Repair if the emergency renders the Covered Product unfit for a person to use because of defects that immediately endanger the health and safety of the user of the Covered Product. In the case of an Emergency Repair, You will not be required to obtain Our prior authorization for service. Only in the case of an Emergency Repair, You may directly contact a qualified and insured service provider to obtain service in the event of failure or malfunction of a Covered Product, for which repair is prescribed under this Agreement. Upon completion of the service, the service provider must provide You an itemized invoice for the charges. You should try to find a service provider who will charge a fair and reasonable cost for parts and labor as You will be responsible for paying the service provider directly for the services rendered, including the Deductible or similar charge up to the Deductible, as well as all costs over and above those charged during normal business hours such as overtime. You will then submit the itemized invoice to Us for reimbursement up to the limit of coverage under this Agreement. Please call the Administrator at 866 4 RIIDES or (866 474-4337) to find out the best way to submit the paid invoice to Us; or, You may submit the paid invoice by mailing it to the Administrator at 0000 Xxxx Xxxxx, Xxx Xxxx, XX 00000 with an explanation of the emergency, when it occurred, Your name, Your account number and Your contact information. We may need to contact You for further information.

This warranty will not be honored by such manufacturer or wholesale company. The Oklahoma Department of Insurance does not review commercial service warranty contract language. Coverage afforded under this contract is not guaranteed by the Oklahoma Insurance Guaranty Association. CANCELLATION - The cancellation section of this contract is deleted in its entirety and replaced with the following: If You cancel this Contract within the first thirty (30) days and no claim has been authorized or paid, we will refund the entire Contract purchase price. If You cancel this Contract after the first thirty (30) days, or have made a claim within the first thirty (30) days, we shall retain ten percent (10%) of the unearned pro rata Contract purchase price or twenty-five dollars ($25), whichever is less. If we cancel this Contract, one hundred percent (100%) of the Contract purchase price will be refunded.

Coverage afforded under this Service Agreement is not guaranteed by the Oklahoma Insurance Guaranty Association. The cancellation provision in this Service Agreement is hereby deleted and replaced with the following: You may cancel this Service Agreement at any time by providing notice to Us at the address indicated above. We may cancel this Service Agreement only in the event of fraud or material misrepresentation, Your failure to pay any amount due hereunder, violation of any of the terms and conditions set forth herein, or as required by any regulatory agency. If You cancel within the first forty- five (45) days of the date You purchased this Service Agreement, You shall receive a full refund of the purchase price. If You cancel after the first forty-five (45) days of coverage, or have made a claim within the first 45 days, You shall receive a 90% pro rata refund of the purchase price less a cancellation fee equal to the lesser of $25 or 10% of the unearned pro rata premium. In the event this Service Agreement is cancelled by Us, return of the purchase price shall be based upon one hundred percent (100%) of the unearned pro- rata purchase price. Refunds owed and not paid within thirty (30) days shall include a ten percent (10%) penalty fee per month. OREGON ONLY: Pursuant to ORS 36.600-36.370 if claim settlement cannot be reached, the parties may elect arbitration by mutual agreement at the time of the dispute after the claimant has exhausted all internal appeals and can be binding by Your consent. Arbitration shall take place in Oregon, unless another location is mutually agreed to, and takes place under Oregon laws.
