Change Request Form definition

Change Request Form means the document provided under this Schedule which shall be used to acknowledge the amendments agreed by both Parties of the Change;
Change Request Form means a document in substantially the same form as that in Schedule 5 or such other form approved by the Customer. Claim means any allegation, cause of action, liability, claim, proceeding, suit or demand of any nature, whatsoever arising, and whether present or future, fixed or unascertained, actual or contingent and whether at Law, under statute or otherwise. Commencement Date means the date specified as such in the Order Form.
Change Request Form means the form set out at Schedule 2 (Change Request Form) of this Framework Agreement; Charges means the charges specified in the relevant Order payable by the Customer for the supply of the Services and Products by SCC;

Change Request Form the template form as set out in 0 of Schedule 3 Change Control Procedure to be used by either Party when submitting a Change Request to the other Party;
Change Request Form a written request for a Change which is submitted by one party to the other pursuant to the Change Control Procedure and in the form set out in Appendix B. Customer Personal Data: any Personal Data comprised in the Customer Data. Customer Policy: those policies as indicated in advance by the Customer. Customer Site: any premises occupied by the Customer at which it receives the Managed Services. Customer-site Equipment: any equipment located or to be located on a Customer Site but controlled or to be controlled exclusively by the Supplier as part of the Managed Services. Customer’s Operating Environment: the Customer’s computing environment (consisting of hardware, software and internet networks) that is to be used by the Customer in connection with its use of the Services and which interfaces with the Supplier’s System in order for the Customer to receive the Services, but excluding the Customer-site Equipment. Customer's Project Manager: the member of the Customer Account Team. The Customer's Project Manager at the Effective Date is named in the Statement of Work.
