SITE SUPERVISOR RESPONSIBILITIES. Provide work assignments that will enhance the student’s knowledge of psychology and/or related applied professions, and provide directly supervised paraprofessional experiences (e.g., assessment, therapy, case management, treatment planning, etc.) Provide the student with sufficient work assignments to both fill the agreed number of practicum hours and provide a worthwhile learning experience. “Busy-work” will be kept to a minimum. Meet directly with the student for at least one hour each week to provide supervision and feedback regarding work assignments. Provide a written evaluation (using form to be provided) of the student’s performance at the end of the practicum. Contact the practicum coordinator (Xx. Xxxxx) as soon as possible if questions arise or the student’s performance appears not to be adequate. List any additional site supervisor responsibilities: Practicum coordinator responsibilities: Be available to discuss and assist with administrative problems, if such arise. Provide final approval of the Practicum Agreement, making recommendations for amendments if necessary. Provide feedback on assignments specified in the course syllabus. Manage administrative aspects of course requirements, assigning course credits, and grades. Student responsibilities: Perform the tasks listed on this agreement, assigned by the site supervisor, and/or listed in the course syllabus. Inform the practicum coordinator immediately of any problems arising related to the practicum. SITE SUPERVISOR RESPONSIBILITIES. As a site supervisor for this internship, I agree to: • Clearly discuss the requirements of the internship with the student intern; • Work with the student to complete on-site goals, duties and learning objectives; • Provide ongoing supervision and feedback to the student on his/her performance; • Communicate with the faculty supervisor and meet with him/her during the site visit; • Complete both the periodic and final evaluation forms in a timely manner.

Discuss the evaluation with the student prior to submitting it to the Faculty/Instructor. • Provide student with opportunities to observe operations of the Site, including staff meetings. • Provide opportunities for the student to network and interact with staff, managers, and volunteers in the professional field. • Provide safe working conditions and the necessary workspace, tools, equipment, and supplies for the stu- dent to complete the internship. Provide a background check if required. • Contact the Internship Coordinator in a timely manner if attendance or other work performance issues arise. • Ensure that no regular employees are displaced by the student. UNIVERSITY FACULTY / INSTRUCTOR RESPONSIBILITIES: • Review and provide feedback on weekly assignments and final evaluation. • Evaluate student’s performance on a pass/no pass basis and enter grade in university system. • Assist student in selecting an internship site and developing learning goals. • Approve the Internship Agreement for completeness (Internship requirements, description and learning goals) • Preauthorize CINE 404 Internship. • Mediate student issues related to the internship. SITE SUPERVISOR RESPONSIBILITIES. Provide AmeriCorps Member(s) access to communications devices while they are serving on site (computer, telephone, etc.) and other tools necessary for regular communication with KY READY Corps staff, as well as equipment and space necessary to successfully perform site tasks; • Reimburse the AmeriCorps member for all travel cost incurred at the request of the service site according to service site policy; and • Provide service site orientation and training as needed; • Provide KY READY Corps staff and member with a specific Assignment Description within the first week of member service and ensure that AmeriCorps member(s) only provide service activities as per that document; • Ensure your member does not perform any AmeriCorps Prohibited Activities listed in this document; • Schedule sufficient time for the member to serve their required hours; • Track and approve members hours of service via the time sheet interface; • Approve member(s) teleservice (service off site when necessary), as well as member(s) written work plan while teleserving; • Assist and support member in collecting required data; • Meet with your member(s) weekly to provide them with the mentoring, skill development, supervision, day-to-day scheduling, resources, assistance and guidance necessary to fulfill the requirements of the position; • Be accessible to the KY READY Corps member(s) for day to day questions; • Establish safety guidelines that ensure the well-being of the AmeriCorps members. Ensure that member receives adequate information on site safety and organization’s safety policies and procedures; • Participate in the AmeriCorps performance evaluation process at least once per member term of service by documenting on the evaluation form provided by KY READY Corps staff; • Assist the member in identifying and achieving personal goals; • Support KY READY Corps and AmeriCorps by allowing the member to wear AmeriCorps gear and to follow AmeriCorps guidelines and directives, including required reporting; • Allow members to attend required training, recognition and service projects with Serve KY or CNCS; • Participate in site visits with KY READY Corps staff; and • Immediately communicate problems that arise with the AmeriCorps member to the KY READY Corps Program Director.
