Life and Health Insurance. 1. The Employer will continue to provide to full-time Employees the same life and health insurance coverage at the same cost as is provided by the Xxxxxx County Board of Commissioners to its other County Employees during the term of this Agreement; provided that the Employer will provide a minimum of thirty-five thousand ($35,000) life insurance coverage to eligible Employees.
Life and Health Insurance. I. Life Insurance -- Employee only (Disability Waiver of Premium) Term life insurance shall be provided and paid for by the Board for all professional employees. The amount of coverage of such term insurance for each professional employee shall be two times basic annual salary rounded to the nearest one thousand dollars to the maximum of one hundred thousand dollars ($100,000). There shall be a maximum life insurance benefit for retirees of forty thousand dollars ($100,000).

During the term of this AGREEMENT the TOWN will maintain a life insurance policy in an amount equal to one times the base yearly wage [hourly rate times two thousand one hundred eighty four (2,184) hours] for each member of the Association, at no cost to the employee.

The Company, in its discretion, may apply for and procure in its own name and for its own benefit, life insurance on the life of the Employee in any amount or amounts considered advisable by the Company, and the Employee shall submit to any medical or other examination and execute and deliver any application or other instrument in writing, reasonably necessary to effectuate such insurance. The Company may provide health insurance, including major medical coverage, for the Employee. All insurance provided to Employee shall be in such form and provide such coverage as may be determined by the Board of Directors or the Compensation Committee of the Board of Directors.
Life and Health Insurance. Section 5 of the Agreement is hereby amended to read in its entirety as follows:
Life and Health Insurance. The Company may provide health insurance, including major medical coverage, for the Employee. All insurance provided to Employee shall be in such form and provide such coverage as may be determined by the Board of Directors or the Compensation Committee of the Board of Directors. Until such time as the Company provides health insurance coverage to the Employee, commencing on January 1, 2014, the Company shall reimburse the Employee for expenses incurred by him for his personal health insurance premiums, in an amount not to exceed $1,500 per month. Reimbursement requests must be timely submitted by the Employee and, if timely submitted, reimbursement payments shall be made to the Employee as soon as administratively practicable following such submission, but in no event later than the last day of the Employee’s taxable year following the taxable year in which the expense was incurred. In no event shall the Employee be entitled to any reimbursement payments after the last day of the Employee’s taxable year following the taxable year in which the expense was incurred. This paragraph shall only apply to in-kind benefits and reimbursements that would result in taxable compensation income to the Employee.”
Life and Health Insurance. The employees covered by this agreement shall receive the same life and health insurance as provided to all other City employees. Employees’ dependents, as defined by the City's insurance carrier, are to be included in the medical coverage. In cases of denied or disputed claims, the City will on request investigate the dispute, and if the City determines that the claim is valid, the City shall provide reasonable assistance to the employee in making an appeal to the insurance carrier, provided the terms of the applicable policies shall govern eligibility for any benefits and no disputes shall be subject to the grievance procedure of this agreement. The employees shall be given, upon request, the current explanation of the insurance program and coverage. Employees shall pay twenty-five percent (25%) of dependent coverage for health care costs. The dependent health care costs shall be calculated based on the city's procedure in effect on May 1, 19941. The employee shall be allowed to participate in the City wellness program.
Life and Health Insurance. The Company will establish, maintain and administer a Life and Health Insurance Plan for each eligible hourly paid employee and eligible dependents which shall consist of and be limited to the benefits outlined herein.
Life and Health Insurance. 42.1 LIFE INSURANCE A death benefit in the amount of Thirty Thousand Dollars ($30,000) shall be paid in accordance with the policy to the designated beneficiary of an employee upon the employee's death, providing such death occurs after the employee has completed thirty

The Corporation shall provide health insurance, including major medical coverage, for the Employee, as described in the “DG Medical Options” schedule, a copy of which has been provided to the Employee, which schedule may be modified from time to time in the discretion of the Corporation. The Corporation shall be obligated to bear the expenses of the coverage contemplated by the previous sentence, to the extent provided in such schedule.
