Fees and Insurance. The fee for your first appointment is determined by a Client Service Specialist during the intake process. At the first appointment, you and your therapist will establish the ongoing appointment fee. Payment is expected at the time of your appointment. Care and Counseling accepts cash, checks, MasterCard, Discover, and Visa. Care and Counseling accepts Medicare and some insurance as in-network, and other insurance as out-of-network. This varies per the individual therapist / provider, so please discuss this further with your therapist. If you select to use your insurance, we will assist you in answering basic questions about your benefits, as well as submit claims on your behalf. You will need to provide your current insurance identification care at the time of your initial appointment. Your plan may include deductibles, co-insurance, and co-pays. Ultimately, you are responsible for payment and understanding your insurance policy. The standard fee is $190 for an initial appointment, and $175 for ongoing appointments. For those whose household income does not support the standard fee, a sliding-fee scale or subsidy through our Client Assistance Fund may be available. Eligibility for these adjustments is required and depends on available resources. Please discuss this information further with your therapist. Care and Counseling is a not-for-profit counseling center, and timeliness of payments is important. Overdue accounts may result in formal collection procedures. Fees and Insurance. For those not using insurance the fees are as follows: $140 per initial assessment hour for a psychologist $120 per treatment hour for a psychologist $120 per initial assessment hour for a counselor $110 per treatment hour for a counselor $50 per treatment hour for a group session I understand that I am responsible for any and all fees incurred by me or a family member as a result of treatment provided by JFS of Richmond. I understand that I am responsible for contacting my insurance carrier to see if outpatient mental health benefits are covered. I am responsible for obtaining any required referrals and authorizations prior to my visit. If I do not have a referral at the time of my scheduled appointment, I will be required to self- pay or reschedule. It is my responsibility to understand my plan’s benefits. If I am covered under an in network insurance plan that provides benefits for behavioral and mental health, JFS of Richmond will file a claim on my behalf. I understand that all applicable co-payments and co-insurance will be applied and payment is expected at the time of service. Once insurance claims have be processed, if the amount collected at the time of the appointment is less than the amount that insurance determines is due, a monthly bill will be sent out that will inform me of the balance due. Payment is due within 10 days of receiving a bill. If my account remains delinquent, JFS of Richmond reserves the right to discontinue services until full payment is received or a payment arrangement has been made JFS of Richmond also may refer my account to a collection agency if 60 days delinquent. In addition, I agree to be responsible for all costs of collection, including all attorney fees and court costs. If there was overpayment, JFS of Richmond will credit my account or refund me promptly. I understand that insurance reimbursement is a contract between me and my insurance company. JFS of Richmond cannot accept responsibility for collecting a disputed insurance claim. If the claim is denied or not paid within 60 days, a bill will be sent to me and I am responsible for contacting my insurance company to research the reason and remedy. Fees and Insurance. The billing department of Gladstone Psychiatry & Wellness will assist in checking the patient’s insurance coverage and benefits; however, the patient is responsible for verifying that their policy covers the treatment provided and understanding any financial responsibility for which they may be liable beyond the restrictions of their policy. A $75.00 short-notice cancellation/re-schedule fee will be charged directly to the patient if the telemedicine or online counseling appointment is not cancelled within 24 hours of the scheduled appointment time. A $75.00 no-show fee will be charged directly to the patient if the patient fails to participate in the scheduled telemedicine or online counseling appointment. There is a risk of loss of service or troubles with connectivity when participating in telemedicine or online counseling. No fee will be charged to the patient if this occurs at the fault of Gladstone Psychiatry & Wellness.
