FEE FOR SERVICE. In providing its services and operations, Contractor will maintain compliance with requirements of the AOD Policy and Procedure Manual including additions and revisions, incorporated by reference herein. The maximum length of stay is set by the specific funding source and an extension beyond a 90 day period may be granted only by written approval from the Alcohol and Other Drug Services (AOD) Administrator, pursuant to a Contractor’s written request outlining and justifying the client’s clinical need. Reimbursement will be approved only for clients who referred through the formal referral process outlined in the AOD Policy and Procedure Manual.
FEE FOR SERVICE. A method of making payment to health care Providers enrolled in the Medicaid program for the provision of health care services to Members based on the payment methods set forth in the State Plan and the applicable policies and procedures of the Division.

Such fees shall be set forth in the annual Extended Employment Services Provider Agreement. FEE FOR SERVICE. 3.01 The Provider agrees to pay CAC the fees and costs invoiced (plus G.S.T.) for the provision of the accreditation on-site review and accreditation outcome, as set out in the schedule of fees.

The MDHHS shall reimburse the CMHSP, in accordance with MDHHS-approved budgets and Medicaid reimbursement policies, for xxxxxxxx submitted by the CMHSP for each beneficiary with a MDHHS approved Children's Waiver. The CMHSP will be reimbursed based on the xxxxxxxx submitted, as this program shall not be pre-paid.
FEE FOR SERVICE. If you do not want to use your insurance or SCC is not in your plan, you will be billed directly for services provided to you. We accept credit, cash, or checks. COURT ORDERED: Please note that if you are recommended to SCC by an attorney or the courts for services, often times the insurance company will not cover those charges, so we will ask for full payment at the time of service. FEE FOR SERVICE. 10.1 The Town shall pay the Society an annual fee based on full cost recovery for the services specified in this Agreement for each year of the term of this Agreement (“Fee for Service”). The Society shall submit to the Town by July annually, its request for Fee for Service based on full cost recovery for the next fiscal year, together with supporting documentation as determined necessary by the Society’s Executive Director. Payment of the Fee for Service approved by the Town shall be made in twelve equal payments per year, no later than the fifteenth of each month, provided that the information for the previous quarter required under section 8.0 has been provided to the Town by the Society.

For the duration of the Work Plan referred to in Section 2.5 above, Kolltan shall pay Spirogen for any services provided pursuant to the Work Plan at an hourly rate equal to $[**] per hour. Spirogen shall invoice Kolltan once monthly for all amounts due for services provided pursuant to the Work Plan, which invoice shall set forth sufficient detail related to the work performed and time incurred for Kolltan to confirm its payment obligations under this Agreement. All invoices shall be due within [**] days of receipt, except that Kolltan may withhold any amounts disputed in good faith. Spirogen shall at its own cost make and retain, during the Term and for a period of [**] thereafter, records (in form and substance reasonable acceptable to Kolltan) of all services performed by Spirogen pursuant to the Work Plan and all costs related thereto. Spirogen shall permit representatives of Kolltan to audit, inspect, and copy [**] per year during regular business hours, during the Term and for [**] thereafter, the books, records, and accounts of Spirogen to the extent that such books, records, and accounts pertain to or otherwise relate to the performance of such services, including any costs incurred for or charged to Kolltan. Kolltan shall pay for any such audit unless the audit reveals that Kolltan overpaid fees due pursuant to this Article during the period of the audit by [**] percent ([**]%) or more, in which case Spirogen shall pay the cost of the audit.
FEE FOR SERVICE. Xxxxxxx Xxxxx fees are for professional services relating to the recruitment and/or selection and/or presentation of candidates for employment, and/or consulting services. Xxxxxxx Xxxxx services shall be deemed to be completed to the satisfaction of the Client when a candidate who is presented by Xxxxxxx Xxxxx accepts an offer of employment with the Client.
