In an HMO plan, you have access to a network of providers who collaborate to bring you personal, efficient care. You will choose a Primary Care Physician (PCP) who is your first point of contact and manages your care. Your PCP is part of a group of Physicians called a Medical Group. Your PCP can refer you to Participating Providers in your Medical Group for specialized care and assist with other care needs. The Access+ HMO offers a wide choice of Physicians, Hospitals, and other Health Care Providers and includes special features such as Access+ Specialists.

Examples of Your Employer in a sentence

  • Your Employer must provide you with a copy of the Notice of Cancellation, Rescission or Nonrenewal.

  • Your Employer is solely responsible for notifying their Employee of the availability and duration of family leaves.

  • Your Employer is responsible for a monthly payment to Blue Shield for health care coverage for the Subscriber and any enrolled Dependents.

  • Blue Shield can cancel coverage if: • You are no longer eligible for coverage in this plan; • Your Employer fails to meet Blue Shield’s Employer eligibility, participation, and contribution requirements; • Blue Shield terminates this plan; or • You or your Employer commit fraud or intentional misrepresentation of material fact.

  • Your Employer is responsible for notifying Blue Shield in writing of the Subscriber’s termination or reduction of hours of employment within 30 days of the qualifying event.
