Compensation Report. If this Agreement is a sole-source, public-private agreement or if the Grantee, through this agreement with the State, annually receive 50% or more of their budget from the State or from a combination of State and Federal funds, the Grantee shall provide an annual report, including the most recent IRS Form 990, detailing the total compensation for the entities' executive leadership teams. Total compensation shall include salary, bonuses, cashed-in leave, cash equivalents, severance pay, retirement benefits, deferred compensation, real-property gifts, and any other payout. The Grantee must also inform the Department of any changes in total executive compensation between the annual reports. All compensation reports must indicate what percent of compensation comes directly from the State or Federal allocations to the Grantee.

Total compensation shall include salary, bonuses, cashed- in leave, cash equivalents, severance pay, retirement benefits, deferred compensation, real-property gifts, and any other payout. The Subgrantee must also inform the County of any changes in total executive compensation between the annual reports. All compensation reports must indicate what percent of compensation comes directly from the State or Federal allocations to the Subgrantee.
Compensation Report. Upon request of the Union, the Health and Safety Committee shall investigate and report as soon as possible on the nature and cause of the accident or injury. An employee who injured during working hours and is required to leave for treatment, or is sent home for such injury shall receive payment for the remainder of the shift at his regular rate of pay without reduction from sick leave, unless a doctor or nurse states that the employee fit for further work on that shift. Transportation to the nearest physician or hospital for employees requiring medical care as a result of an accident shall be at the expense of the Corporation. Compensation Report. In cases where Client manufactures the product and pays a royalty to the Supplier, and also in cases where due to special circumstances the Supplier is paid directly by Client, within thirty (30) days after the end of each month, Client shall send to GIS a report setting forth the total number of units purchased or manufactured for sale and total dollar amount of sales and a statement showing the total Compensation to be paid to GIS for such period plus the royalty to be paid to Supplier, if applicable (the “Royalty/Compensation Report”). Unless GIS or Supplier object to the accuracy of the Royalty/Compensation Report, GIS shall sign and return the report to Client together with GIS’s invoice for the Royalty and Compensation amounts stated in the Royalty/Compensation Report. Client shall pay GIS’s invoice by wire transfer to the bank account designated by GIS within the time period negotiated by the parties, but no longer than 60 days. Notwithstanding GIS’s acceptance and return of the Royalty/Compensation Report, GIS shall not be deemed to have waived any right to later audit Client as provided in this Agreement or otherwise. Client’s obligation to produce the Royalty/Compensation Report each month and pay the royalty and Commission shall survive the termination of this Agreement and shall continue so long as Client continues to have an obligation to pay compensation to GIS under this Agreement.
