Xxxxxx of Insurance. Employees, who voluntarily waive the insurance benefits set forth above, will be paid 25% of the premium cost of the program(s) in which the employee is currently eligible. Eligible employees hired after the date of July 1, 2005 shall receive 25% of the premium cost of the lowest cost program(s). Payment will be made in two installments – January 31st and June 30th. Once an employee voluntarily waives insurance coverage, the employee may re-enroll upon proof of any of the life events as designated by the Department of Insurance. Employees hired after July 1st of any year who elect not to take insurance coverage(s) or employees who terminated employment prior to June 30th shall have the payment prorated based on the number of months the employee was employed.

Xxxxxx of Insurance. If 11 or few bargaining unit members opt out of health insurance coverage, they shall each receive a $1,250 waiver bonus. If more than 11 members opt out, each of those OAPSE members shall receive a $2,500 waiver bonus. When the parties meet to negotiate a successor agreement to this contract, the health insurance waiver bonus shall be $1,250 unless negotiated otherwise. When both spouses are employees of the District, they shall be covered by a family plan under the name of one of the spouses. Upon the other spouse's application, he/she shall receive a $1,250 cash payment at the end of the contract year.
